MCM2在宫颈病变组织中的表达及意义

摘　　要：目的：通过检测微小染色体维持蛋白2（MCM2）在不同级别宫颈病变组织中的表达，探讨其在宫颈病变发展中的意义。方法：收集宫颈石蜡标本122例，采用免疫组化SP法检测MCM2在不同级别宫颈病变组织、不同细胞学及HPV结果中的表达情况。结果：MCM2阳性着色于细胞核。MCM2在宫颈组织中阳性表达分别为正常组织1例（5．00％）、CINI9例（45．00％）、CINⅡ～Ⅲ32例（76．19％）、浸润癌40例（100．00％），其表达阳性率随着宫颈病变恶性程度的增加而升高，各组间比较差异有统计学意义（χ^2=60．65，P〈0．001）。在细胞学水平，MCM2的阳性表达分别为无宫颈上皮内瘤变（NILM）1例（5．00％）、不典型鳞状上皮细胞（ASCUS）6例（50．00％）、低度鳞状上皮内病变（LSIL）13例（59．09％）、不典型鳞状上皮不除外高度鳞状上皮内病变（ASC-H）26例（86．67％）、高度鳞状上皮内病变（HISL）36例（94．74％），其表达阳性率随着细胞学恶性程度增加而升高，各组间比较差异有统计学意义（χ^2=55．61，P〈0．001）。73例HPV阳性患者中，MCM2表达阳性率为79．45％（58／73），而49例HPV阴性患者中，MCM2表达阳性率48．98％（24／49），两组比较差异有统计学意义（χ^2=12．35，P〈0．05），且MCM2的表达与HPV感染呈正相关（r=0．75）。结论：MCM2随宫颈病变恶性程度的增高表达升高，在宫颈癌呈高表达。MCM2的高表达可能与宫颈癌发生有关。Objective：To discuss the expression MCM2 in different cervical lesion and to explore the clini- cal significance of MCM2 in the development of cervical disease. Methods：122 cases cervical paraffins sam- ples were collected,and the expression of MCM2 were detected by immunohistochemistry. Results：The lo- cation of MCM was in the nucleus. The positive rates of MCM2 were normal 1 （5.00%） 、CIN Ⅰ 9 （45.00%）, CIN Ⅱ - Ⅲ32 （76. 19% ） ,cancer 40（ 100.00% ）, respectively, and the expression of MCM2 became higher gradually with the malignant degree increased, and the difference among the groups was statistically signifi- cant（ X2 = 60. 65, P 〈 0. 001 ） ; and NILM 1 （5.00%） 、 ASCUS 6 （ 50. 00% ） 、 LSIL13 （ 59. 09% ） 、ASC-H 26 （86.67%） 、HISL 36（94.74% ）, respectively, and the expression of MCM2 became higher gradually with the malignant degree increased, and the difference among the groups was statistically significant （X2 = 55. 61, P 〈0. 001 ）. The MCM2 positive rate was 79.45% in 73 HPV positive patients while 48.98% in 49 HPV neg- ative ones, there was significant difference on the MCM2 positive rate between the two groups（X2 = 12.35, P 〈 0.05）, and the expression of MCM2 was positively related with HPV infection （r = 0.75）. Conclusions= The expression of MCM2 became higher gradually with the malignant degree increased. The high expression of MCM2 may be related to the onset of cervical cancer.
