摘  要:目的探讨三维步态分析在膝关节周围恶性肿瘤患者术后康复治疗中的应用价值。方法采用三维步态分析方法比较15例肿瘤型人膝关节假体置换术患者(A组)和15例正常对照人(B组)的时间-空间参数、膝关节运动学参数以及力学参数。结果与B组相比,A组步长、步速、步频、患侧单腿支撑时间、承重反应期最大屈膝角度、支撑相最大伸膝角度、摆动相最大屈膝角度、矢状面第1次及第2次最大伸膝力矩均减少,而矢状面最大屈膝力矩增大(P<0.05)。结论三维步态分析可以精确测量肿瘤型膝关节假体置换术后患者的膝关节时间-空间参数、运动学参数及力学参数,从而为患者提供康复指导。Objective To investigate the application value of three-dimensional gait analysis in the rehabilitation of patients with malignant tumor around the knee. Methods The time-distance parameter, knee kinematical parameter and mechanical parameter were compared by three-dimensional gait analysis in 15 cases with cancer around the knee underwent total knee replacement(group A) and 15 normal controls (group B). Results Compared with group B, the step length, step speed, step frequency,affected single leg supporting time, largest knee extension, largest knee flexion in swing phase, largest knee flexion in loading response, the 1st and 2nd extension torque of sagittal plane were significantly decreased, while max flexion torque of sagittal plane was increased in group A (P〈0. 05). Conclusion The three-dimensional gait analysis can accurately measure the changes of time-distance parameter, knee kinematical parameter and mechanical parameter in the patients with cancer around the knee underwent total knee replacement, which may provide the patients with rehabilitation guidance.
