摘 要:目的探讨颞叶癫痫(TLE)患者工作记忆(WM)的神经学基础及其认知改变。方法通过血氧水平依赖性功能磁共振成像(BOLD-fMRI)技术,采集15例TLE患者和15例健康志愿者行参数性数字2-back WM任务时的BOLD-fMRI数据,SPM5软件后处理并进行比较分析。结果与正常对照组相比,TLE患者N-back WM任务执行行为学表现较弱;两组主要激活脑区均以额、顶叶为主,TLE患者组WM任务激活脑区范围、感兴趣区激活体素和激活强度均较小。结论 (1)WM的神经学基础是以额叶为主导,额叶、顶叶、颞叶等多个脑区共同构成的WM网络;(2)TLE患者WM相关脑功能区及其WM网络存在不同程度的受损,导致WM认知功能障碍。Objective The study was aimed to explore the neural basis of WM and its cognition change in temporal lobe epilepsy (TLE) patients with BOLD-fMRI technique and digit 2-back WM task. Methods The data of 15 TLE patients and 15 normal subjects were acquired with BOLD-fMRI by performing a digit 2-back WM task. Statistical analysis were performed using SPMS. Results The performance for N-back WM task was lower in TLE patients compared with normal subjects. The frontal lobe and the parietal lobe were main activated brain areas in the two groups, but the range of WM task activated area, the number of activated voxels and the average t value of the RO1 were smaller in TLE patients. Conclusion ( 1 ) The neural basis of WM is a distributed network including frontal lobe, parietal lobe, and temporal lobe, and the frontal lobes play a leading role; (2) There is impairment in WM relevant brain areas and WM network in TLE patients, which leads to WM dysfunction.
