^(18)氟-脱氧葡萄糖PET/CT对结节病的影像学诊断分析

摘　　要：目的评价18F-FDG PET/CT在肺结节病中的诊断价值。方法对10例(男4例,女6例)经病理证实(6例)和随访确诊(4例)的结节病患者,进行回顾性临床资料总结。根据18F-FDG PET/CT的图像特征,结合病灶累及的部位、分布特点、大小、摄取标准值(SUV)特点进行分析。结果 PET/CT显示淋巴结累及者共10例,其中双肺门及纵隔淋巴结累及者10例,锁骨上窝淋巴结累及者3例,腹膜后淋巴结累及者6例,盆腔淋巴结累及者4例。结外脏器累及者9例:包括肺内结节及片状阴影5例,肝脏累及者3例,双侧髂骨及骶骨累及者1例。所有病灶代谢均明显增高者8例,部分病灶代谢明显增高者2例。结论18F-FDG PET/CT可灵敏、准确地反映结节病的全身病灶分布范围和病灶活性。病灶分布于双侧肺门和纵隔,呈结节形串珠状连结为结节病18F-FDG PET/CT的特征表现。根据PET/CT影像特点并结合临床能提高结节病诊断的准确性。Objective To evaluate the value of ^18F-fluorodeoxyglucose （FDG）PET/CT images of patients with sarcoidosis. Methods A retrospective analysis of the clinical data was made on the patients （4 males and 6 females） with sarcoidosis, which were verified with ^18F-FDG PET/CT imaging features, coupled with pathological sites involved, distributing features, lesion size and stand- ardized uptake value（SUV）. Results （ 1 ） PET/CT results indicated that 10 cases were involved in lymph nodes, of which 10 cases were respectively involved in pulmonary hilus lymph nodes and mediastinal lymph nodes. Three cases were involved in supraclavicular fossa lymph nodes, 6 cases were involved in retroperitoneal lymph nodes and 4 cases were involved in pelvic cavity lymph nodes. （2） Nine cases （9/10） ： including 5 cases of pulmonary lymph nodes, and 3 cases of liver involvement, and 1 case of bilateral iliac and sa- cral bone involvement. There were 8 patients, whose metabolism was enhanced significantly in all the loci and there were 2 patients, whose metabolism was enhanced significantly. Conclusion 18F-FDG PET/CT could sensitively and accurately refect the distribution of sarcoidosis throughout the body and the activity of loci. The foci were widely distributed in the bileral lung hili and mediastinum. Com- bined with clinical manifestation, the PET/CT imazin~ could imDrove the accuracv in the diaznosis of sarcoidosis.
