摘　　要：目的调查并分析某三甲医院抗菌药物不合理使用情况,以提高医院临床处方质量。方法采用回顾性调查方法,随机抽取某三甲医院门诊住院普通处方12 000份,按照卫生部颁发的《抗菌药物临床研究指导原则》《医院处方点评管理规范(试行)》《专项整治活动方案》等对医院抗菌药物不合理用药表现进行调查分析。结果 12 000份处方中有2 562份使用抗菌药物,占收集处方的21.35%。不合理用药处方258份,占抗菌药物处方的10.07%,主要不合理用药类型为用药时间过长、联合用药不合理、用药时间错误、用法用量不合理等。结论某三甲医院各临床科室使用抗菌药物基本合理,但仍存在不合理现象,应加强监管,以促进抗菌物药的合理应用。Objective To investigate the irrational use situation of antibacterial drugs for improving the prescription quality in a Class 3A hospital. Methods By adopting the retrospective survey method,12 000 pieces of common prescriptions in the outpatient and inpa- tient departments of a class 3A hospital were randomLy extracted and performed the investigation analysis on the irrational use manifes- tations of antibacterial drugs according to the Guidance Principle for Clinical Researches on Antibacterial Agents,the Hospital Prescrip- tions Comments Management Standard （on trial） and the Special Modification Plan for Use of Antibacterial Agents, etc. issued by the Ministry of Health. Results Of 12 000 pieces of prescription,2 562 pieces of prescription used the antibacterial agents,accounting for 21.35% of collected prescriptions. There were 258 pieces of irrational medication prescription,accounting for 10.07% of the antibacte- rial prescriptions. The major irrational use types included too long medication duration, irrational combination use, wrong administration time,irrational dosage and usage,etc. Conclusion Antibacterial agents use is basically rational in this class 3A hospital,but the irra- tional use phenomena still exist. The supervision should be strengthened to promote the rational use of antibacterial agents.
