耐碳青霉烯类铜绿假单胞菌产金属β-内酰胺酶相关基因型的分布

摘　　要：目的 研究本地区患者对碳青霉烯类抗生素耐药的铜绿假单胞菌产金属酶检出情况及基因型分布特征.方法 本研究收集2012年2月～2013年2月共151株铜绿假单胞菌,分离出对碳青霉烯类抗生素耐药的铜绿假单胞菌47株.使用IMP/EDTA纸片法筛选产金属酶的耐药表型,PCR技术检测编码金属酶的IMP、VIM、SPM和GIM 4种基因型.PCR反应产物进行纯化后,进行核酸电泳.结果 47株对碳青霉烯类抗生素耐药的铜绿假单胞菌中,IMP/EDTA纸片法检测金属酶表型阳性21株,占44.6％.PCR检测金属酶基因型阳性14株,占29.8％,其中IMP型阳性11株,占78.6％,VIM型阳性3株,占21.4％.未检测出SPM和GIM型金属酶.结论 池州地区产生的金属酶同时存在IMP和VIM两种基因型,其中以IMP亚型为主,少部分为VIM亚型,分布科室主要为ICU,其次为神经外科.产金属酶是本地区对碳青霉烯类抗生素耐药的铜绿假单胞菌的重要耐药机制,对碳青霉烯类耐药的铜绿假单胞菌产金属酶的监测十分重要.Objective To investigate the genotype of metallo-β-1actamases（MBL） produced by carbapenem resistant Pseudomonas aeruginosa in Chizhou. Methods 47 strains resistant to imipenem or meropenem were collected in the Metropolitan Hospital of Chizhou from December 2012 to November 2013. Isolates were further evaluated for MBL pro- duction by another screening methods. Molecular screening for blaVIM ,blaIMP,blaSPM, and blaGIM was carried out us- ing primers targeting the conserved regions of MBL-coding sequence. The PCR products were analyzed by nucleic acid elec- trophoresis. Results Of the 47 carbapenem-resistant P. aeruginosa strains, 21（44.6%） were MBL positive using EDTA/ IMP methods;14（29.8%） strains were PCR-positive; 11（78.6%） were positive for blaIMP genes; and 3（21.4%） were positive for blaVIM genes. All isolates were negative for blaSPM and blaGIM. Conclusion There were two genotypes（IMP, VIM） of metallo-β-lactamases produced by carbapenem-resistant P. aeruginosa in Chizhou city. IMP-producing strains were dominant in ICU and neurosurgery, which coincides with VIM-producing strains. The production of MBL is one of the important mechanisms of carbapenem-resistant P. aeruginosa. It is crucially needed to monitor the roduction of MBL.
