Anatomic locking plates for distal tibia fracture using minimally invasive plate osteosynthesis technique

摘  要:目的探讨解剖锁定钢板微创治疗胫骨下段骨折的临床效果。方法应用解剖锁定钢板微创治疗胫骨下段骨折87例,对其术后进行随访,定期进行x线检查及踝关节功能评定。结果87例患者中,有65例获得12~46个月(平均24.7个月)随访,其中45例已行内固定取出。有2例因钢板断裂,再次开放手术,行锁定钢板内固定,获得痊愈后经行内固定取出;另有2例于术后8月获得愈合;有2例出现小腿屈肌腱粘连。AOFAS踝关节功能评分系统对术后1年的踝关节功能进行评分:优50例,良6例,一般4例,优良率为92.1%。结论经皮解剖锁定钢板微创治疗胫骨下段骨折,减少了软组织的剥离,保护了骨块的血运,降低了骨折不愈合率及感染率,踝关节获得了良好的功能。Objective To explore the clinical effect of minimally invasive plate osteosynthesis (MIPO) in distal tibia fracture. Methods From February 2009 to June 2012, 87 cases of distal tibial fracture were treated with MIPO technology. Patients were followed up after operation, and their regular X-ray examination and ankle joint function assessment were evaluated. Results Sixty-five cases were followed up for a period of 12 -46 months ( average, 24.7 months). Among them, 45 cases were removed of internal fixation, 2 had the second time surgery for rupture of the plate, 2 had a delayed fracture healing 8 months after operation, 2 developed flexor tendon adhesion. One year after the operation, 50 cases were excellent, 6 cases good. and 4 cases fair according to AOFAS standard for the evaluation of curative effect of ankle joint function. Conclusion MIPPO can reduce the split of soft tissue, protect the blood supply, and decrease no-healing rate and infection rate, thus result in a better ankle joint function for patients with distal tibia fracture.
