椎动脉缺血型颈椎病的MRA改变及临床意义

摘　　要：目的 :用三维动态MRA无创成像技术探讨椎动脉缺血型颈椎病MRA表现及其临床意义。方法 :眩晕者共 76例 ,均行MRA椎动脉检查。结果 :测量出椎动脉正常值 ,左侧为 (4.1± 0 .79)mm ,右侧为 (3 .2± 0 .83)mm。根据MRA表现将椎动脉缺血型颈椎病分为 3型 :Ⅰ型 (椎动脉受压型 ) ,Ⅱ型 (椎动脉硬化型 ) ,Ⅲ型 (椎动脉畸形型 )。通过椎动脉MRA成像特点能够对受累椎动脉作出精确、定性诊断。对 5例椎动脉Ⅰ型患者行椎动脉松解术后 ,MRA显示椎动脉的上述改变已改善或消失 ,疗效优良。结论 :MRA是椎动脉缺血型颈椎病的一重要筛检诊断工具 。Objective:The 3D MRA of 76 cases suspected as vertebral artery insufficient of cervical spondylosis were studied.Methods: All cases were measured with vertebral artery.The diameter of left vertebral artery is 4.1±0.79mm.That of right vertebral artery is 3.2±0.83mm.Results: The cervical spondylosis of the vertebral artery insufficiency is divided into three types according to the MRA images:typeⅠ(The verteral artery compression type):It shows the artery's pressure、trusion、sinuous or obstruction,et al.typeⅡ(The vertebral arteriosclerosis type):It shows string-beads.Artery occlusion,centripetal arteriostenosis,et al.typeⅢ(The vertebral artery malformation type):It shows that the difference of two vertebral arteries size is large or the numbers of artery are not symmetry、Ring sinuous etc.MRA can get the high level diagnosis and qualitative diagnosis.after 5 cases of typeⅠwere performed with uncinate joint resection,MRA shows the change was modified or disappeared.Conclusion: So MRA is of significant value in the prognosis、diagnosis and the choice of treament.
