2型糖尿病血瘀证患者血压变异性的临床研究

摘　　要：目的探讨血压变异性(BPV)对2型糖尿病血瘀证患者的临床意义。方法选取2型糖尿病患者非血瘀证58例,血瘀证122例,予动态血压(ABP)监测,比较2组的临床指标及ABP、BPV参数。结果血瘀证组各时段血压标准差大于非血瘀证组,差异有统计学意义(P<0.05);非血瘀证组勺型趋势比例大于血瘀证组,差异有统计学意义(P<0.05);血瘀证组各时段的收缩压负荷均大于非血瘀证组,差异有统计学意义(P<0.05);筛查出吸烟、低密度脂蛋白胆固醇(LDL-C)、昼夜趋势为主要危险因素,差异有统计学意义(P<0.05)。调整混杂因素后,勺型昼夜趋势的相对危险度(OR值)为0.006,差异有统计学意义(P<0.001)。结论非勺型昼夜趋势可能为糖尿病血瘀证的独立危险因素。Objective To explore the clinical significance of blood pressure variability （BPV） in patients with blood stasis syndrome of type-2 diabetes mellitus （T2DM）. Methods Among patients with T2DM, fifty-eight cases of non-blood stasis syndrome and one hundred and twenty-two cases of blood stasis syndrome were selected. The monitoring of ambulatory blood pressure （ABP） was applied. The clinical index as well as ABP and BPV were compared between two groups. Resttlts The standard deviation of blood pressure in blood stasis syndrome group at each time point was significantly higher than that in the non-blood stasis syndrome group （ P 〈 0. 05 ）. The percentage of dipper-like trend in the non-blood stasis syndrome group was significantly higher than that in the blood stasis syndrome group （ P 〈 0. 05 ）. The load of systolic blood pressure in blood stasis syndrome group at each time point was significantly higher than that in the non-blood stasis syndrome group （ P 〈 0. 05 ）. Smoking, low-density lipoprotein-cholesterin （ LDL-C ）, day-night trend were screened as dangerous factors （ P 〈 0. 05 ）. After adjusting of mixed factors, the relative risk （RR） of dipper-like day-night trend was 0. 006 （ P 〈 0. 001 ）. Conclusion The non-dipper-like trend of day and night could be an independent dangerous factor in patients with blood stasis syndrome of T2DM.
