中西医结合预防神经外科手术后肺炎的临床研究

摘　　要：目的观察中西医结合预防神经外科手术后患者肺炎的效果。方法将200例神经外科手术后进人ICU的患者随机分为观察组和对照组。2组均采用常规治疗及抗生素治疗，观察组根据不同中医证型口服5～7天中药预防术后肺炎汤剂，观察2组住院期间（或术后28天）肺炎发生率、肝肾功能损伤率及死亡率等。结果共194例患者完成观察，观察组肺炎发生率较对照组低，差异有统计学意义（P〈0．05）；观察组28天死亡率、60天死亡率、肝肾功能异常率虽较对照组有一定改善，但差异无统计学意义（P〉0．05）。结论中西医结合可降低神经外科手术后患者医院获得性肺炎发生率，安全有效。Objective To observe the effects of integrated traditional Chinese and western medicine on the prevention of post- neurosurgery pneumonia. Methods Two hundred cases that received ICU after neurosurgery were randomly divided into an observation group and a control group. The routine treatment including antibiotics was applied in both groups, and the observation group was treated with oral administration of TCM decoction according to different types of TCM diagnosis for 5 to 7days. The occurrence rate of pneumonia, injury of liver-kidney function, and mortality rate were observed in two groups during hospital stay （or within 28 days after surgery） . Results A total of 194 cases completed the observation. The occurrence rate of pneumonia in the observation group was significantly lower than that in the control group （ P 〈 0. 05 ） ; the mortality rate within 28 days and within 60 clays as well as rate of liver-kidney injury were improved in the observation, which were not significant compared with those in the control group （ P 〉 0. 05 ）. Conclusion The integrated traditional Chinese and western medicine could prevent the occurrence of hospital-acquired pneumonia after neurosurgery, which is safe and effective.
