儿童哮喘控制测试在门诊定期随访管理中的应用价值

摘　　要：目的探讨儿童哮喘控制测试(C-ACT)在门诊定期随访管理中,辅助评价儿童哮喘控制水平的应用价值。方法选择56例符合哮喘诊断标准的患儿,于其接受哮喘控制药物治疗后1、2、3、4个月时,逐月门诊随访观察及评估。随访时由哮喘专科医生依据全球哮喘防治创议制订的儿童哮喘控制水平分级标准,对患儿哮喘控制水平进行评估,并由专科门诊护士指导患儿及家长使用C-ACT评价其哮喘控制水平和症状程度。结果 4次随访结果均显示,CACT评分与哮喘专科医生评估在患儿哮喘控制分级水平上比较,差异有统计学意义(P<0.05),其一致性比率分别为67.86%,69.64%,75.00%,80.36%。对224总例次数据分析显示,C-ACT得分在专科医生评估的不同控制水平患儿之间比较,差异有统计学意义(P<0.05)。结论在哮喘专科门诊的定期随访管理中应用C-ACT,能够提高患儿及家长评估哮喘控制水平的可靠性。Objective To evaluate the application value of Children Asthma Control Test （C-ACT）as the adju- vant assessment tool for the measurement of asthma control level in the follow up management of asthma children. Methods Fifty-six children diagnosed as asthma were recruited in this study. The children＇s asthma control level was measured once a month both by the physicians according to the grading standard developed by Global Initia- tive for Asthma（GINA） and by the children and parents using C-ACT guided by pediatric nurses. Results The asthma control level rated by the physicians was significantly different from the results rated by the children and parents using C-ACT （P〈0.05）. The application of C-ACT in consistent ratios were 67.86%,69.64%,75.00% and 80.36% in the four time points,respectively. Conclusion The application of C-ACT in regular follow-up management of asthma children can improve the reliability of assessment of asthma control level by children and parents.
