自发性早产孕妇血清妊娠相关蛋白-A和抑制素-A表达水平的研究

摘　　要：目的：探讨正常孕妇各孕周血清妊娠相关蛋白-A（PAPP—A）和抑制素-A（inhibin—A）的变化情况及其与自发性早产发病的相关性。方法：采用镧系元素解离增强荧光免疫检测技术（DELFIA）检测孕20～41^＋6周妇女外周血PAPP—A浓度1085例，其中正常孕妇880例，自发性早产孕妇205例；采用酶联免疫吸附法（ELISA）检测外周血inhibin—A浓度896例，其中正常孕妇691例，自发性早产孕妇205例。统计分析自发性早产孕妇及正常孕妇外周血清PAPP—A和inhibin—A的中位数差异。结果：正常孕妇血清PAPP—A和inhibin—A水平值随孕周的增加而不断增高，至孕晚期达到最高水平，但二者之间无相关性；自发性早产组PAPP—A水平显著低于相应孕周正常孕妇组的中位数，inhibin—A水平较相应孕周正常孕妇组无明显变化。结论：正常孕妇血清PAPP—A和inhibin—A水平随妊娠进展而逐渐上升，PAPP—A与自发性早产发病相关。Objective： To explore the changes of pregnancy associated plasma - A （ PAPP - A） and inhibin A in normal pregnant women and its correlation with spontaneous preterm birth. Methods： DELFIA was used to detect the concentration of PAPP- A in peripheral blood of 1 085 pregnant women of 20 -41 ＋6 gestational weeks, including 880 normal pregnant women and 205 pregnant women of spontaneous preterm birth; ELISA was used to detect the concentration of inhibin A in peripheral blood of 896 pregnant women, including 691 normal preg- nant women and 205 pregnant women of spontaneous preterm birth. The differences of medians of PAPP - A and inhibin A in peripheral blood between pregnant women of spontaneous preterm birth and normal pregnant women were analyzed statistically. Results： The levels of A and inhibin A in normal pregnant women increased with extension of gestational weeks, which peaked during the third trimester of pregnancy, but there was no correlation between PAPP - A and inhibin A ; the median of PAPP - A level in pregnant women of spontaneous preterm birth was significantly lower than that in normal pregnant women during the same gestational week; compared with the normal pregnant women during the same gestational week, the median of inhibin A level in pregnant women of spontaneous preterm birth didnt change significantly. Conclu- sion： The levels of PAPP - A and inhibin A in normal pregnant women increase with extension of pregnancy, PAPP - A is correlated with the occurrence of spontaneous preterm birth.
