静脉穿刺在CT增强扫描中的应用及失败原因分析

摘　　要：目的总结CT增强扫描静脉穿刺要点,分析失败原因,以提高静脉穿刺成功率。方法收集2011年10月至2012年3月来我院行CT增强扫描的患者3 750例,使用BD密闭式静脉留置针,型号为18～20 G,选择肘部较大血管进行穿刺。结果一次穿刺成功3 700例,成功率为98.67%;一次穿刺失败50例,失败原因包括患者血管条件差10例,患者不配合7例,操作人员技术失误18例,心理状态不佳导致失败15例,失败率为1.33%。结论 CT增强静脉穿刺时必须严格遵守静脉穿刺程序,熟练掌握穿刺要点,培养良好的心理素质,增强责任心,可提高静脉一次性穿刺成功率。Objective To summarize the main points of vein puncture in enhanced CT scan, and analyze the reasons of failure and improve the vein puncture success rate. Methods 3 750 patients from October 2011 to March 2012 underwent CT contrast scan, with BD closed intravenous catheter, model 18-20 G, the elbow larger vessels were selected. Results 3 700 cases with one-time puncture success, and success rate was 98.67%; 50 cases with once puncture failure, the reasons for failure include patients with poor vascular conditions in 10 cases, 7 patients do not co- ordinate well, the operator technical errors in 18 cases, poor mental state leads to failure in 15 cases, and the failure rate was 1.33%. Conclusion We need strictly observed, master puncture point, cultivate good psychological quality, and enhance the sense of responsibility, to increase the venous puncture success rate in CT enhancement procedures.
