Pemberton髂骨截骨术治疗发育性髋关节脱位73例临床分析

摘　　要：目的探讨Pemberton髂骨截骨术治疗发育性髋关节脱位的临床价值。方法回顾性分析2010年1月至2012年8月在我科手术治疗的发育性髋关节脱位患儿资料,73例患儿共计80髋,均经Pemberton髂骨截骨术治疗,观察术后并发症发生率,并比较治疗前后CE角、AI角及髋臼深度的变化,分析Pemberton髂骨截骨术的临床疗效及安全性。结果随访12～44个月,73例患儿均获得随访。疗效评定按Mckay标准,优为57髋(71.3%),良为15髋(18.8%),优良率90.1%。治疗后CE、AI角度及髋臼深度均较治疗前获得显著改善(P<0.05),差异具有统计学意义。结论 Pemberton髂骨截骨术在发育性髋关节脱位治疗中效果肯定,安全,并发症少,是治疗儿童发育性髋关节脱位的重要术式。Objective To explore the clinical application of pemberton joint capsule around the iliac osteoto- my in the treatment of congenital dislocation of the hip. Methods From 2010 January to 2012 August, 73 patients （80 hips） with congenital dislocation of the hip in our department were studied retrospectively. The 80 hips with congenital dislocation were received pemberton joint capsule around the iliac osteotomy treatment. The CE comer, AI angle and depth of acetabulum were compared before and after treatment. Results All patients were followed up for 12-44 months. Based on Mckay clinical grading standard, the clinical effect was excellent in 57 hips （with the excellent rate of 71.3%） and good in 15 hips （with the good rate of 18.8%）. The excellent and good rate was 90.1%. The CE angle, AI an- gle and the acetabular depth improved significantly after treatment, compared before treatment. Conclusion The cu- rative efficacy of pemberton joint capsule around the iliac osteotomy for congenital dislocation of the hip is favorable and relatively safe, which is worthy of clinical application.
