超声引导针灸治疗腋路连续臂丛阻滞临床效果分析

摘　　要：目的:探讨超声引导针灸治疗腋路连续臂丛阻滞的临床效果。方法:选取本院收治的上臂、手臂、手掌桡侧、手腕手术的患者80例随机分为研究组和对照组,研究组在在极泉、曲池、合谷、小海、阳溪等穴进针,并给予电刺激,频率为2 Hz,初始电流1 mA,然后在超声引导下定位行腋路连续臂丛阻滞,对照组取腋动脉搏动最上点做皮内小泡,行解剖定位臂丛阻滞。比较两组麻醉效果、术中心率、呼吸、血压和血氧饱和度变化及并发症发生情况。结果:研究组阻滞起效时间显著短于对照组,维持时间显著长于对照组,麻醉优良率为97.5%,显著高于对照组77.5%(P<0.05)。两组术中心率、呼吸、血压和血氧饱和度比较无统计学差异(P>0.05)。研究组并发症发生率显著低于对照组(P<0.05)。结论:超声引导下腋路连续臂丛阻滞可以缩短神经组织起效时间,提高麻醉效果,患者并发症发生率较低,效果较好。Objective ：To investigate the clinical effect of ultrasound- guided continuous axillary brachial plexus block. Meth- ods ：80 patients of the upper arm operation, arm operation, hand operation or radialis wrist operation in our hospital were random- ly divided into study group and control group. The study group was needled at Jiquan, Quchi, Hegu, Xiaohai, Yangxi and given the electrical stimulation with the frequency of 2Hz and the initial current of lmA continuous axillary brachial plexus block in ultra- sound. The control group was made the anatomic location of brachial plexus block, The effects and the complications of two groups were compared. Results ： The onset time of study group was shorter than that of the control group and the maintain time was longer than that in the control group. The excellent and good rate was 97.5%, significantly higher than that of the control group （77. 5% ） （P 〈 0.05 ）. Two groups＇intraoperative heart rate, respiration, blood pressure and oxygen saturation had no significance （P 〉 0. 05 ）. The incidence of complications was significantly lower than that of control group （ P 〈 0. 05 ）. Conclusion ： The ultrasound - guided continuous axillary brachial plexus block can shorten the onset time of neural tissue and improve the effect of anesthesia with a low incidence of complications and good results.
