摘 要:目的了解并分析北京市某功能社区职业人群血脂流行趋势及与累积糖尿病发病风险的关系,为功能社区职业人群的健康管理提供依据。方法采用重复测量方差分析和多元Logistic回归对北京某功能社区26—60岁373名职业人群2010--2012年的血脂和空腹血糖重复测量数据进行分析。结果上述职业人群2010--2012年这三年总胆固醇(TC)异常分别占3.49%、4.83%和5.90%,甘油三酯(TG)异常占20.9l%、22.79%和21.98%,高密度脂蛋白胆固醇(HDL—C)异常占13.40%、29.76%和21.72%,低密度脂蛋白胆固醇(LDL—C)异常占0.54%、2.41%和3.49%,空腹血糖异常占1.07%、1.34%和2.95%。男性Tc平均水平随年份和年龄变化呈升高的趋势(P〈0.01)。不同年份TG平均水平男性均高于女性(P〈0.01),HDL—C平均水平男性均低于女性(P〈0.01)。Logistic回归分析显示,高龄者发生糖尿病的风险是低龄者发生糖尿病的风险的1.165倍,TG异常者发生糖尿病的风险是TG正常者发生糖尿病的风险的2.987倍。结论某功能社区职业人群血脂总体上有上升的趋势,高甘油三酯水平是糖尿病发病风险的相关因素,多年的血脂连续监测数据能对糖尿病发病风险的预测提供更多信息。Objective To explore the serum lipid trends and to assess the association between the change in serum lipid level and the cumulative incidence of diabetes mellitus among occupational population in a functional community of Beijing, and to provide suggestions for health management. Methods One way repeated measures ANOVA and multiple logistic regression analysis were carried out using data from year 2010 to 2012, which was repeated measurement data of blood lipid and fasting plasma glucose from 373 occupational subjects aged 26 to 60 in a functional community of Beijing. Results From 2010 to 2012, the abnormal number of total cholesterol (TC) was accounted for 3.49%, 4. 83% and 5.90% respectively ; abnormal triglyceride (TG) 20. 91% , 22. 79% and 21.98% respectively ; abnormal high density lipoprotein cholesterol ( HDL - C) 13.40%, 29.76v and 21.72% respectively; abnormal low density lipoprotein cholesterol ( LDL - C) 0. 54% , 2. 41% and 3.49% respectively; and abnormal fasting plasma glucose (FPC) 1.07%, 1.34% and 2. 95% respectively. The average male TC increased with year and age(P 〈0. 01 ). The mean TG level was significantly higher in men in three years than that of women (P 〈 0. 01 ) , and the mean HDL -C level in men was lower than that of women(P 〈 0. 01 ). Logistic regression analysis showed that the risk of diabetes mellitus among the elders was 1. 165 times of younger people, and the risk of diabetes mellitus among people with abnormal TG was 2. 987 times of those with normal TG. Conclusion High serum lipid level was common in occupational population. TG level is associated with cumulative incidence of diabetes, and the change of TG level in several years could predict the risk of diabetes.
