摘  要:目的观察宫腔镜技术对绝经后子宫出血早期诊断的准确性。方法选择114例自然绝经1年以上子宫出血的门诊患者,先进行门诊宫腔镜检查,得出镜下诊断,并在宫腔镜定位下取宫腔组织做病理学检查。对镜下诊断为良性宫腔占位病变者安排住院行宫腔镜电切手术。统计宫腔镜检查镜下诊断与病理学诊断各种疾病的例数,并分析镜下诊断与病理学诊断的符合率。结果 114例绝经后子宫出血患者,宫腔镜下诊断萎缩性子宫内膜8例,子宫内膜炎6例,子宫内膜息肉70例,子宫黏膜下肌瘤18例,可疑子宫内膜癌4例,镜下诊断与病理学诊断符合率为96.5%。结论宫腔镜操作简便,能直接观察宫腔情况,根据宫内病变的大体形态特点初步作出镜下诊断,宫腔镜下定位取材行病理诊断,镜下诊断与病理学诊断符合率高。Objective To observe the early diagnosis of postmenopausal uterine bleeding and observe its accuracy. Methods 114 cases with bleeding after natural menopause more than lyear in outpatient department were selected,they were given hysteroscopie examinations firstly,and hysteroscopic diagnosis was maded.Endometrial biopsy located under hysteroscopy was carried out,and the uterine cavity tissue was send for pathological examination.Patients with hystero- scopic diagnosis of benign uterine cavity lesions were hospitalized for hysteroscopie surgery.The numbers of cases of various disease diagnosed by hysteroscopy and pathology were counted separately,and the conincidence rate of the above was analyzed. Results Among the 114 cases with postmenopausal uterine bleeding,8 cases were diagnosed with atrophic endometrium,6 cases with endometritis,70 cases with endometrial polyps,18 cases with submucosal myoma,4 cases with suspected endometrium carcinoma,the conincidence rate of hysteroseopie and pathological diagnosis was 96.5%. Conclusion Hysteroseopy is simple and convenient,which can directly observe the uterine cavity, therefore lead to preliminary diagnosis according to the morphologie characteristics of intrauterine lesions,the conincidence rate of hysteroscopic and pathological diagnosis is high when endometrial biopsy is located by hysteroscopy.
