摘 要:目的 :通过建立兔髂动脉粥样硬化 (AS)模型 ,观察球囊血管成形术 (BA )对血浆肿瘤坏死因子α(TNF-α)、内皮素(ET)水平及局部血管组织 ET免疫反应性 (ET- IR)的影响。 方法 :2 0只新西兰雄性大白兔经高脂饲料喂养至少 8周造成AS,然后进行髂动脉 BA ,18只兔获得成功。检测了 BA前、即刻及 2 4h后血浆 ET和 TNF-α水平 ,ET采用放射免疫法测定 ,TNF-α采用生物活性法测定。同时 ,应用免疫组织化学技术评价血管组织 ET- IR。 结果 :BA前后即刻 :血浆 ET水平 (pg/ml)分别为 76 .4± 13.5 8和 92 .6 7± 11.38(P<0 .0 5 ) ;2 4h后恢复至扩张前水平 77.13± 12 .87(P>0 .0 5 ) ;而血浆 TNF-α水平 (U /m l)分别为 31.35± 6 .2 3和 5 6 .2 6± 7.37(P<0 .0 5 )。 2 4h后血浆 TNF-α水平 (33.41± 6 .79)与扩张前相比无明显差异(P>0 .0 5 )。BA后局部髂动脉血管壁 ET- IR较未经 BA的髂动脉明显增加。结论 :BA后血浆 ET、TNF-α水平升高及局部血管壁 ET- IR增加可能与局部血管内膜损伤有关 ,提示 ET、TNF-α可能参与了Objective: To assess the effect of balloon angioplasty on circulating endothelin (ET) and TNF α levels and tissue endothelin in experimental atherosclerosis in rabbits. Methods: After 20 New Zealand rabbits had a high cholesterol diet for at least 8 weeks, successful balloon angioplasty was performed in rihgt iliac arteries in 18 rabbits. Circulating levels of ET and TNF α were measured before as well as immediately and 24 h after balloon angioplasty. Tissue endothelin immunoreactivity in atherosclerotic iliac artery wall after balloon angioplasty was assessed by immunohistochemical technique. Results: Plasma levels of ET and TNF α were significantly increased immediately after ballon angioplasty (76.40±13.58)pg/ml vs (92.67±11.38) pg/ml and (31.35±6.23) U/ml vs (56.26±7.37) U/ml, respectively ( P <0.05) .There was no change in plasma ET and TNF α levels 24 h after balloon angioplasty (77.13±12.87) pg/ml vs (76.40±13.58) pg/ml and (33.41± 6.79) U/ml vs (31.35±6.23) U/ml, respectively ( P >0.05). Tissue endothelin immunoreactiuvity was markedly increased in right iliac artery wall after balloon angioplasty than that in opposite iliac artery wall. Conclusion: The increase of plasma ET, TNF α levels and tissue ET IR in iliac artery wall after balloon angioplasty may be associated with the injury of local vascular intima, suggesting that ET and TNF α may take part in the coronary constriction and the development of coronary restenosis after percutaneous transluminal coronary angioplasty.
