阿托伐他汀联合曲美他嗪治疗慢性心力衰竭的疗效观察

摘　　要：目的 探讨阿托伐他汀联合曲美他嗪治疗慢性心力衰竭的疗效.方法 回顾性分析2008年1月至2012年12月我院收治的165例慢性心力衰竭患者,随机分为联合治疗组（85例）和常规治疗组（80例）.联合治疗组在地高辛、利尿剂、硝酸脂类药物、血管转换酶抑制剂及β受体阻滞剂等常规治疗基础上联合阿托伐他20～40 mg每晚1次和曲美他嗪20mg每天3次口服治疗,总疗程为6个月.观察两组患者临床症状和体征的变化,采用超声心动图测定左室舒张末期内径（LVEDD）、左室收缩末期内径（LVESD）、E/A比值和左室射血分数（LVEF）的变化,并行6 min步行距离测定和血清FFA的测定.结果 联合治疗组心功能改善总有效率为91.8％,常规治疗组总有效率81.3％,两组比较差异有统计学意义（P＜0.05）.治疗6个月后,两组LVEDD、LVESD、LVEF、E/A、6min步行距离测定、血清FFA水平均比治疗前明显改善,差异有统计学意义（P＜0.05）;与常规治疗组比较,联合治疗组改善更为明显（P＜0.05）.结论 在常规治疗的基础上联合阿托伐他汀和曲美他嗪可以进一步改善慢性心力衰竭患者的心功能,并且具有良好的安全性.Objective To investigate the therapeutic effects of atorvastatin combined with trimetazidine in chronic heart failure.Methods A retrospective analysis of 165 patients with chronic heart failure in our hospital from January 2008 to December 2012 was performed.Enrolled patients were randomly divided into combined treatment group （85 cases） and conventional treatment group （80 cases）.On the basis of the conventional treatment with digoxin,diuretics,nitric acid lipid drugs,ACEIs and β-blockers,atorvastatin 20-40 mg every night and trimetazidine 20 mg three times daily were combined in the combination therapy group for 6 months.Changes of clinical symptoms and signs,left ventricular end-diastolic diameter （LVEDD）,left ventricular systolic diameter （LVESD）,E/A ratio and left ventricular ejection fraction（LVEF） measured by echocardiography were observed in the two groups before and after the therapy.Six minutes walking distance and level of serum FFA were determined.Results Total effective rate of the improvement of heart function in the combination therapy group was 91.8％,and the total effective rate of the conventional treatment group was 81.3％.The comparison was statistically significant （P＜0.05）.After 6 months of treatment,LVEDD,LVESD,LVEF,E/A,6 min walking distance was significantly improved in the two groups.The difference was statistically significant（P＜0.05）.Compared with the conventional treatment group,the combination therapy group improved more significantly （P＜0.05）.Conclusion On the basis of conventional therapy,the therapy combined with atorvastatin and trimetazidine can further improve cardiac function in the patients with chronic heart failure,and has a good security.
