微粒子酶免疫法检测血清5项肿瘤标志物对于消化系统肿瘤和良性疾病的诊断价值分析  被引量:5

摘  要:目的探讨在消化系统良性疾病以及6种肿瘤患者β2微球蛋白(β2-MG)、糖链抗原carbohydrate antigen 19-9(CA19-9)、carbohydrate antigen 125(CA125)、癌胚抗原(CEA)、甲胎蛋白(AEP)5种标志物应用微粒子酶免疫法(MEIA)检测的诊断价值。方法选取消化系统良性疾病患者350例及恶性肿瘤患者360例,对其血清中CA19-9、CA125、CEA、AEP以及β2-MG 5种标志物浓度进行检测,对比检测结果。结果上述5项指标阳性率显著增高,良性患者与胆囊癌、胰腺癌、结直肠癌、胃癌患者之间阳性率差异具有统计学意义(P〈0.05);消化系统肿瘤患者阳性率整体提升幅度为19.8%-40.6%;消化系统良性病与肿瘤诊断特异性分别为:β2-MG100.0%-53.8%、CA19-9 87.0%-58.2%、CA125 100.0%-50.0%、CEA 100.0%-81.3%、AFP 100.0%-62.3%。结论鉴别消化系统良性疾病与肿瘤,β2-MG、CA125、AFP、CEA以及CA19-9有重要意义,联合上述5项标志物检测有利于检测灵敏性的大幅提升。Objective To investigate the clinical diagnostic value of serum β2-microglobulin( β2-MG),carbohydrate antigen 19-9( CA19-9),carbohydrate antigen 125( CA125),CEA and AFP for benign diseases and six kinds of tumors of the digestive system by microparticle enzyme immunoassay( MEIA). Methods A total of 350 patients with digestive system benign diseases and 360 patients with digestive system tumors were collected to detect the levels of serum CA19-9,CA125, CEA,AEP and β2-MG. Then the detection results were analyzed and compared. Results The combined detection of the five indicators above can significantly improve the positive rate of digestive system diseases. The positive rates of patients with benign diseases and that of patients with gallbladder carcinoma,pancreatic cancer,colorectal cancer and gastric cancer showed significant statistical difference( P 0. 05); The overall positive rate of patients with digestive system tumors was improved by 19. 8% - 40. 6%; In diagnosis of digestive benign diseases and tumors,the specificity was 53. 8% - 100. 0% for β2-MG,58. 2% - 87. 0% for CA19-9,50. 0% - 100. 0% for CA125,81. 3% - 100. 0% for CEA and 62. 3% - 100. 0% for AFP,respectively. Conclusion The β2-MG,CA125,AFP,CEA and CA19-9 had important significance and high clinical application value in the differential diagnosis of digestive benign diseases and tumors. Combined detection of the five markers above can significantly improve the sensitivity of detection.
