摘 要:目的通过观察不同保存期悬浮红细胞过滤前后各项指标的变化,评价滤除白细胞的悬浮红细胞质量变化。方法用一次性三联采血袋随机采集30袋400 ml全血,在万级净化间无菌条件下等分成4袋,置于(4±2)℃冰箱中保存。分别在保存期第0 d、7 d、14 d、21 d使用去白细胞滤器过滤并制备悬浮红细胞,留取过滤前后的血样,将其分为未过滤组和过滤组。测定样本血液分析,并分析白细胞滤除率,血小板滤除率,红细胞回收率;测定血浆游离血红蛋白(FHb)和钾离子(K+)含量。结果不同保存期第0 d、7 d、14 d、21 d未过滤组和过滤组悬浮红细胞的Hb、RBC、WBC和PLT数据进行分析比较均无统计学差异;白细胞平均滤除率为99.95%;血小板平均滤除率为94.83%;红细胞平均回收率为92.21%;过滤前后组间血浆游离血红蛋白(FHb)和钾离子(K+)浓度,随着保存时间的延长有增加,经统计学处理差异无统计学意义(P>0.05);未过滤组血浆游离血红蛋白(FHb)和钾离子(K+)浓度在保存21 d明显高于相同时间保存的悬浮红细胞,但差异无统计学意义(P>0.05)。结论不同保存期滤除白细胞的悬浮红细胞质量符合国家血液质量控制标准,能够安全有效输血。Objective To observe the change of each indicator of suspended erythrocytes with different storage life before and after leukocyte depletion by filtration,and to evaluate the quality change of suspended erythrocyte. Methods Thirty bags of 400 ml blood samples were randomly collected from 30 donors using disposable triple blood bag,and then equally divided into 4 bags in ten-thousand-level clean room under grade aseptic conditions,and preserved in( 4 ± 2) ℃ fridge. Erythrocyte suspension was prepared using leucocyte-depletion filter on the 0,7th,14th,21st,28th,35thdays,and the blood samples were collected before and after filtration as unfiltered group and filtration group. The blood routine indicators( Hb,RBC,WBC, PLT),leukocyte filtration rate,platelet filtration rate,red blood cells recovery rate,as well as the contents of free hemoglobin( FHb) and K+were determined in erythrocyte suspension before and after leukocyte filtration. Results The contents of Hb, RBC,WBC,PLT in erythrocyte suspension had no statistical difference between filtration group and unfiltered group on the 0, 7th,14th,21st,28th,35thdays. The average leukocyte filter rate was 99. 95%,the average platelet filtering rate was 94. 83%, and the average recovery rate of erythrocyte was 92. 21%. As the extension of storage time,the contents of plasma free hemoglobin( FHb) and( K+) increased in erythrocyte suspension before and after leukocyte filtration,but there was no obvious difference on statistics( P〉0. 05). While the contents of plasma free hemoglobin( FHb) and( K+) were significantly higher in unfiltered group on the 21stdays than those with the same storage time,but there was no obvious difference on statistics( P〉0. 05). Conclusion The quality of suspended erythrocyte remains in line with the national blood quality control standard during different storage time after leukocyte depletion by filtration,safe and effective for clinical blood transfusion.
