摘 要:目的:探讨100kVp条件下使用碘克沙醇(270mg I/mL)行肺动脉CTA检查的可行性。方法:54例连续患者行肺动脉CTA检查,分为两组:100kVp组共27例,使用对比剂碘克沙醇(270mg I/mL);120kVp组27例,使用对比剂为碘帕醇(370mg I/mL)。两组使用对比剂的总量均为35mL。记录2组的CT剂量指数(CTDI)、剂量长度乘积(DLP),并计算有效剂量(ED)和体型特异性剂量估计(SSDE)值。对图像质量进行主观评分,并测量每例患者肺动脉各段的CT值,对主观评分和测量结果进行统计学分析。结果:两组患者的性别、年龄和身体质量指数(BMI)的差异均无统计学意义(P=0.785、0.416和0.311)。100kVp组使用对比剂的总碘量为9.45g,120kVp组为12.95g。100kVp组CTDIvol、DLP、ED和SSDE值均低于120kVp组,差异均有统计学意义(P<0.05)。两组图像质量均符合诊断要求,主观评分的差异无统计学意义(P>0.05)。两组肺动脉各段CT值的差异无统计学意义(P>0.05)。结论:使用100kVp管电压、碘克沙醇(270mg I/mL)行肺动脉CTA检查,图像质量能够满足临床诊断需求,同时降低了患者的辐射剂量及对比剂量。Objective:To evaluate the feasibility of using iodixanol (270rag I/mL) with 100kVp technique in CT pul- monary angiography (CTPA). Methods:57 contiguous cases had CTPA were divided into 2 groups,Group A included 27 patients using iopamidol 370mg I/mL,35ml,with 120kVp technique;Group B included 27 patients using iodixanoI 270mg I/mL, 35mL with 100kVp technique. The CT radiation dose index(CTDI), dose length product (DLP) were recorded, effective dose (ED) and size specific dose estimate (SSDE) were calculated. Subjective gradation of image quality were done,the CT values of different segments of pulmonary artery of each patient were measured, statistical analyses were performed between these two groups. Results:No significant statistical difference was existed in sex, age and body mass index (BMI) of these two groups (P=0. 785,P=0. 416 and 0. 311 respectively). The total amount of iodine in Group 13 was lower than that of Group A (9.45g vs 12.95g). The CTDIvol,DLP,ED and SSDE of Group 13 were lower than that of Group A,with signifi- cant statistical difference (P〈0.05). The image quality of the two groups met the requirement of diagnosis, no significant statistic difference was existed in subjective gradation of the two groups (P〉0.05). No statistical difference between two groups was assessed in the CT value of various segments of pulmonary artery (P〉0.05). Conclusion:Using 100kVp technique and iodixanol (270rng I/mL) for CTPA,the image quality could meet the requirement for clinical diagnosis, the radiation dose and the volume of contrast medium could be lowered.
