摘　　要：目的分析早期股骨头坏死(ONFH)患者滑膜中基质金属蛋白酶-13(MMP-13)的表达,探讨其在ONFH病情进展中的作用。方法选取30例早期ONFH患者(坏死组)、30例髋关节骨性关节炎(OA)患者(OA组)、30例股骨颈骨折患者(骨折组),术中取滑膜组织,采用免疫组织化学技术检测滑膜中MMP-13蛋白的表达及滑膜形态学改变,并采用RT-PCR技术检测滑膜中MMP-13 mRNA的表达水平。结果免疫组化结果显示,MMP-13蛋白于坏死组及OA组中表达与骨折组中表达差异有统计学意义(P<0.05),而坏死组与OA组间差异无统计学意义(P>0.05);RT-PCR结果显示坏死组与OA组的MMP-13 mRNA表达量差异有统计学意义(P<0.05)。结论 MMP-13蛋白与mRNA在坏死组与OA组中呈高表达,表明两者可能有着相同的滑膜病变机制,但坏死组mRNA表达较OA组更高,表明滑膜病变的程度可能存在差异。Objective To analyze the expression of matrix metalloproteinase-13 in synovium of early osteonecrosis of femoral head （ ONFH） patients, and to study its effect on progress of ONFH. Methods Selected 30 ONFH patients （osteonecrosis group） , 30 Hip osteoarthritis （OA） patients （OA group） , 30 transcervical fracture patients （fracture group）. Synovium dissected in operations. Investigate expressing of MMP-13 protein and morphological change of synovium by immunohistochemical technology. Investigate expressing of MMP-13 mRNA of synovium by RT - PCR technology. Results Immunohistochemistry results showed statistically significant difference in expression of MMP-13 protein in osteonecrosis group and OA group between fracture group （P 〈 0.05）. No statiscially significant difference between osteonecrosis group and OA group （P 〉 0.05）. RT-PCR results showed statistical significantly difference in expression of MMP-13 mRNA in osteonecrosis group between OA group （P 〈 0. 05）. Conclusion High expression of MMP-13 protein in osteonecrosis group and OA group show they may have the same synovial pathological mechanism. Higher expression of MMP-13 mRNA in osteonecrosis group than OA group show the severity of abnormality synovium is different.
