终末期肾脏病患者血清ucMGP及PIVKA-Ⅱ与桡动脉钙化的相关性

摘　　要：目的探讨血清非活性基质Gla蛋白(ucMGP)及凝血酶原前体蛋白(PIVKA-Ⅱ)与终末期肾脏病(ESRD)患者桡动脉钙化的关系。方法选取45例ESRD住院患者作为病例组,行动静脉内瘘手术时取桡动脉标本,桡动脉标本均行von kossa染色,判断有无钙化,并计算钙化积分。选取同期健康体检人群30例作为对照组。所有研究对象均检测血清ucMGP、PIVKA-Ⅱ、血红蛋白(Hb)、钙(Ca)、磷(P)、白蛋白(ALB)、全段甲状旁腺素(iPTH)、C反应蛋白(CRP)指标。结果病例组血清ucMGP低于对照组(P<0.01),PIVKA-Ⅱ高于对照组(P<0.01)。在病例组中桡动脉钙化与高血P相关,与低ucMGP相关。血清PIVKA-Ⅱ与ucMGP水平及桡动脉钙化无相关性。结论 ESRD患者血清ucMGP低于正常人群,且与桡动脉钙化相关。ESRD患者血清PIVKA-Ⅱ高于正常人群。Objective To determine the correlation between serum concentrations of uncarboxylated matrix Gla protein （ucMGP） and protein induced by vitamine K absence or antagonist- Ⅱ （PIVKA- Ⅱ） and calcification of radial artery in patients with end stage renal disease （ESRD）. Methods 45 patients with ESRD （experimental group） were recruited in nephrology department, while 30 people in good health （control group） were included as control. Radial arteries were partially excised from patients in experimental group through arteriovenous fistula operation, and von kossa-staining was used to judge the degree of calcification. The serum concentrations of ucMGP, PIVKAⅡ ,Hb, Ca, P ,ALB, iPTH and CRP were all included as items of inspection. Results The concentration of ucMGP in experimental group was lower than that in control one, on the contrary, the concentration of PIVKA- Ⅱ was higher in experimental group. In experimental group, the calcification of radial artery was associated with hyperphosphatemia and the lower concentration of ucMGP. There was no significant association between serum concentration of PIVKA- Ⅱ and radial calcification as well as the concentration of ucMGP. Conclusion The serum concentration of ucMGP in patients with ESRD is lower than that in healthy people, and is associated with radial calcification. The serum concentration of PIVKA- Ⅱ in patients with ESRD is higher than that in healthy people.
