摘 要:目的探讨不同剂量红霉素治疗新生儿胃肠道功能紊乱的临床效果。方法选取收治的新生儿胃肠道功能紊乱患儿129例,按照治疗方法分为大剂量组、小剂量组与对照组各43例,大剂量组及小剂量组分别予红霉素10mg·kg-1·次-1、3mg·kg-1·次-1治疗,对照组予等量氯化钠溶液。比较3组患儿肠内营养时间、肠外营养时间、肝功能损害及住院时间。结果 3组肠内营养时间中半量时间、3/4量时间、全量时间及肠外营养时间最短的为大剂量组,其次为小剂量组,最后为对照组,3组间差异有统计学意义(P<0.05)。大剂量组的肝功能损害与小剂量组比较差异无统计学意义,但明显低于对照组,差异有统计学意义(P<0.05);住院时间最短的为大剂量红霉素组,其次为小剂量红霉素组及对照组,3组间差异有统计学意义(P<0.05)。结论采用不同剂量红霉素治疗新生儿胃肠道功能紊乱,可明显缓解患者胃肠道功能紊乱,且大剂量红霉素效果明显优于小剂量。Objective To investigate the clinical effect of different dose of erythromycin on neonatal gastrointestinal dysfunction. Methods 129 cases of neonates with gastrointestinal dysfunction were divided into high-dose, low-dose and con- trol group according to the treatment, there were 43 cases in each group. The high-dose group and low-dose group were respec- tively given erythromycin with 10mg/( kg. times) ,3mg/( kg. times) ,while the control group received normal saline. The enter- al nutrition time, parenteral nutrition time,liver function damage and hospitalization time of 3 groups were compared. Results The half the amount of time,3/4 the amount of time, the whole amount of time in the enteral nutrition and parenteral nutrition time of the high-dose erythromycin group were the shortest, followed by low-dose group, and finally was the control group, there were significant difference among 3 groups ( P 〈0. 05 ). There were no significant difference in the liver dysfunction between the high-dose group and low-dose group, but which was significantly lower than the control group, the difference was significant (P 〈 0.05 ) ;the length of stay of the high-dose group was the shortest,followed by low-dose erythromycin group and the control group, there were significant difference among 3 groups (P 〈 0.05). Conclusion Different doses of erythromycin were able to alleviate gastrointestinal dysfunction in children with gastrointestinal disorders, but large-doses erythromycin is better than low- doses erythromycin.
