抗幽门螺旋杆菌感染治疗特发性血小板减少性紫癜疗效研究

摘　　要：目的 研究单独抗幽门螺旋杆菌（Hp）感染对特发性血小板减少性紫癜患者的血小板恢复情况.方法 选择35例特发性血小板减少性紫癜伴Hp感染,并且血小板大于30×109/L的患者;单独抗Hp感染,以根除Hp感染,观察治疗前后血小板变化,考察Hp根除后,特发性血小板减少性紫癜患者的血小板变化情况.结果 35例患者Hp根治率达到82.9％（30/35）,需要1～2个疗程达到Hp阴性,治疗前血小板中位数37×109/L;Hp转阴后血小板中位数62×109/L; 30例Hp根治后,其中4例（13.3％）达到良好反应,17例（56.7％）达到部分反应.结论 特发性血小板减少性紫癜合并Hp感染,Hp根除后,血小板减少有不同程度改善,甚至部分达到良好反应,这种治疗方法值得推广.Objective To investigate the effect of Helicobacter pylori eradication therapy on the recovery of platelet in patients with idiopathic thrombocytopenic purpura.Methods We studied the effect of H.pylori eradication in 35 H.pylori-positive patients with ITP,and platelet more than 30 × 109/L,by comparing the change of the patient＇s platelet levels before and after treatment.Results By 1 ～ 2 courses of eradicating Helicobacter pylori,the eradication therapy was successful in 35 patients （82.9％）.The median value of patient＇s platelet was 37 × 109/L,62 × 109/L before the treatment and after Hp-negative,respectively.Among of 30 Hp-negative patients,4 patients showed good response,and 17 patients showed partial response.Conclusion After Helicobacter pylori eradication in H.pylori-positive ITP patients,they showed different improvement in platelet count,even good response and partial response,so we advocate the treatment.
