数字减影机X线监视下多靶点臭氧介入联合射频治疗腰椎间盘突出症

摘　　要：目的:观察数字减影机X线(DSA)监视下,多靶点臭氧介入联合射频消融治疗腰椎间盘突出症的临床效果。方法:110例腰椎间盘突出症患者,随机分为靶点臭氧介入加射频组(A组,n=55)和单纯臭氧介入治疗组(B组,n=55)。DSA监视下,在突出椎间盘髓核内、神经根周围及穴位靶点注射臭氧,并对突出髓核进行热凝消融治疗。并分别于术后24小时、1周、1个月、3个月和6个月进行疗效评价。结果:术后24小时、1周、1个月、3个月和6个月的优良率分别为:A组:92.7%、94.5%、92.7%、91.0%、89.1%,B组:87.3%、87.3%、85.4%、83.6%、81.8%。组间对比差异具有统计学意义(P<0.05);术后各时间节点的平均VAS评分A组明显低于B组,差异具有统计学意义(P<0.05),两组均无明显术后并发症发生。结论:DSA监视下多靶点臭氧介入联合消融治疗腰椎间盘突出症疗效优于单纯臭氧注射治疗。Objective：To observe the efficacy of combination treatment with ozone injection and radiofrequency in multi-targets monitored by digital subtraction angiography for lumbar intervertebral disc herniation （LDH）.Methods：110 patients with LDH were randomly divided into group A （combination treatment with ozone and radio frequency,n =55） and group B （Pure ozone treatment,n =55）.Monitoring by DSA,ozone was injected into disc nucleus,around nerve root and other targets in group B; and gave additional thermal coagulation ablation therapy of nucleus pulposus in group A.Treatment effects were evaluated in postoperative 24 h,1 week,1 month,3 months and 6 months respectively.Results：The excellent and good rate of postoperative 24 hours,1 week,1 month,3 months and 6 months are：group A：97.2％,94.5％,97.2％,91.0％,89.1％,and group B：87.3％,87.3％,87.3％,87.3％,81.8％ respectively.There were significant differences between the two groups （P ＜ 0.05）.The postoperative average VAS of each time point in group A was significantly lower than B group （P ＜ 0.05）.There were no significant postoperative omplications in two groups.Conclusion：The treatment effect of combination with ozone injection and radiofrequncy in multitargets monitored by DSA is better than pure ozone treatment for LDH.
