射频消融治疗子宫疾病181例分析

摘　　要：目的射频消融治疗子宫疾病181例分析及临床意义。方法回顾分析2010年10月至2012年3月间在我院行射频消融治疗妇科子宫病变的病例181例,181例病例于超声引导下行射频消融治疗。其中子宫肌壁间肌瘤67例,黏膜下肌瘤73例,功血症子宫内膜增厚41例。结果 181例超声引导下行射频消融治疗中,子宫肌壁间肌瘤67例,子宫黏膜下肌瘤73例,子宫内膜增厚41例。射频治疗后于3个月及6个月行超声复查。其中子宫肌壁间肌瘤67例中25例肌瘤消失,42例缩小约2/3。其中子宫黏膜下肌瘤73例中67例肌瘤消失,6例缩小约2/3。彩超复查瘤体缩小,肌瘤的周边回声增强,血流信号减少。子宫内膜增厚41例中38例内膜较之前薄2/3,3例较之前薄1/3。功能性子宫出血的治疗后,子宫内膜声像恢复正常,阴道流血症状消失。结论自凝刀属于一种高频电滋波,利用其射频生物热效应,使肌瘤细胞死亡,射频消融是创伤小的临床应用之一,在射频消融治疗时超声引导,易于操作准确进行,具有明显临床意义。Objective To radiofrequency ablation in the treatment of uterine diseases: analysis of 181 cases and its clinical significance. Methods From 2010 October to 2012 March in our hospital for radiofrequency ablation in the treatment of gynecological uterine lesions in 181 cases,181 cases in the ultrasound guided radiofrequency ablation. Among 67 cases of uterine intramural myoma,there were 73 cases of submucous myoma,endometrial thickening of uterine bleeding in 41 cases. Results 181 cases of ultrasound guided radiofrequency ablation in the treatment of uterine leiomyoma,67 cases of intramural,73 cases of uterine submucous myoma,endometrial thickening in 41 cases. Radiofrequency treatment after 3 months and 6 months for ultrasonic examination. The intramural uterine myoma in 67 cases in 25 cases of disappearance of tumor,42 cases of reduced about 2 /3. Among them 73 cases of uterine submucous myoma in 67 cases of disappearance of tumor, 6 cases of reduced about 2 /3. Color Doppler ultrasound examination the tumor size,echo myoma peripheral enhancement,reduced blood flow signal. Endometrial thickening in 41 cases,38 cases of endometrial than before the thin 2 /3,3 patients with 1 /3 before thin. Treatment of functional uterine bleeding,uterine returned to normal,symptom of vaginal bleeding disappeared. Conclusions The self coagulation knife belongs to a high frequency electromagnetic wave,the RF biological heat effect,make flesh tumour cell death,radiofrequency ablation is one of the clinical application of small trauma,ultrasound guided radiofrequency ablation,easy operation and accurate,has obvious clinical significance.
