参芪地黄汤联合西药治疗气阴两虚型IgA肾病随机平行对照研究

摘　　要：[目的]观察参芪地黄汤联合西药治疗气阴两虚型IgA肾病疗效。[方法]使用随机平行对照方法,将80例住院患者按抛硬币法随机分为两组。对照组40例强的松,按1mg/kg,1次/d,口服。贝那普利,10mg/次,1次/d,口服。治疗组40例参芪地黄汤(太子参、黄芪各30g,玄参、麦冬各15g,丹皮、女贞子各12g,旱莲草15g,当归、石膏、知母各12g;咽喉肿痛去太子参加金银花15g,牛蒡子10g;血尿明显加大蓟、小蓟各12g,荷叶炭10g;蛋白尿顽固加芡实、金樱子各15g;易感冒加荆芥15g,防风10g,白术15g),1剂/d,水煎200mL,早晚口服。西药治疗同对照组。连续治疗180d为1疗程。观测临床症状、症状积分、检测指标、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈25例,显效15例,无效0例,总有效率100.00%。对照组痊愈8例,显效29例,无效3例,总有效率92.50%。治疗组疗效优于对照组(P<0.05)。症状积分两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。检测指标两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。[结论]参芪地黄汤联合西药治疗气阴两虚型IgA肾病效果显著,值得推广。[Objective]To observe when combined western medicine therapy in ginseng and astragalus qi and Yin deficiency type of IgA nephropathy.[Methods]Using random parallel control method,80 cases of hospitalized patients were randomly divided into two groups according to flip a coin method.The control group of 40 cases of prednisone,press 1 mg/kg,1/d,oral.That split,10 mg/times,1/d,oral.When treatment group(40 cases of ginseng and astragalus(Taizishen,Huangqi each 30g,Xuanshen,Maidong each 15g,Danpi,Nvzhenzi each 12g,Hanliancao 15g,Danggui,Shigao,Zhimu each 12 g;sore throats went to prince for Jinyinhua 15g,Niubangzi10g;hematuria significantly increase Daji,Xiaoji each 12 g,Heyetan 10g,fructus rosae laevigatae proteinuria stubborn add Qianshi,Jinyingzi each 15g,easy to catch a cold and Jingjie 15g,Fangfeng 10g,Baizhu 15g).1/d,boiling water 200 ml,oral sooner or later.Western medicine treatment with the control group.Continuous treatment of 180 d to 1 course of treatment.Observation of clinical symptoms and symptom integral,test indicators,adverse reactions.As a course treatment,judge curative effect.[Results]The treatment group(25 cases cured,15 cases were markedly effective,0 no effect,the total effective rate was 100.00%.Control group cure 8 cases,29 cases had marked effect,3 had no effect,the total effective rate was 92.50%.The treatment group curative effect is better than that of control group(P<0.05).Symptoms integral in the two groups were improved(P<0.05),the treatment group is better than that of control group(P<0.05).Testing index in the two groups were improved(P<0.05),the treatment group improved better than that of control group(P<0.05).When combined western medicine therapy.[Conclusion] ginseng and astragalus qi and Yin deficiency type of IgA nephropathy effect is remarkable,is worth promoting.
