子宫内膜异位症患者内膜组织中uPA与PAI-1蛋白的表达

摘　　要：目的:研究尿激酶型纤溶酶原激活物(uPA)及纤溶酶原激活物抑制物-1(PAI-1)在子宫内膜异位症(EMT)发生发展中的作用。方法:应用免疫组化方法检测40例EMT患者异位内膜与在位内膜及30例对照正常子宫内膜组织中uPA及PAI-1蛋白的表达。结果:uPA和PAI-1蛋白主要定位于细胞浆,在异位内膜、在位内膜、对照子宫内膜的腺上皮细胞及间质细胞均有表达,且在血管内皮细胞亦有表达。uPA和PAI-1蛋白在异位内膜间质细胞的表达均高于在位内膜及对照子宫内膜间质细胞(P均<0.017)。Ⅰ、Ⅱ期与Ⅲ、Ⅳ期异位内膜腺上皮细胞及间质细胞中uPA蛋白的表达差异均无统计学意义(χ2=0.269、1.339,P均>0.05),而PAI-1蛋白在Ⅲ、Ⅳ期异位内膜间质细胞中的表达高于Ⅰ、Ⅱ期(χ2=5.432,P=0.020)。uPA与PAI-1蛋白在异位内膜间质细胞的表达呈正关联(rp=0.311,P=0.039)。结论:uPA与PAI-1蛋白在异位内膜间质细胞高表达,促使细胞黏附、侵袭、转移和生长,参与EMT的发生发展。Aim：To investigate the expressions and functions of uPA and PAI-1 in the etiology and pathogenesis of en-dometriosis.Methods：Immunohistochemistry method was used to detect the expressions of uPA and PAI -1 in 40 cases of ectopic endometrium , 40 cases of eutopic endometrium and 30 cases of normal endometrium .Results： uPA and PAI-1 mainly localized in cytoplasm of stromal and epithelial cells from ecpotic ,eutopic and normal endometrium ,and also were positive in vascular endothelial cells .Endometriotic stromal cells showed a significant increase in the expression rates of uPA and PAI-1 compared with stromal cells of eutopic and normal endometrium （P〈0.017）.There was no statistical sig-nificance between the expression rate of uPA protein in stromal cells and epithelial cells from ectopic endometrium in stageⅠ and Ⅱ, and stageⅢandⅣ（χ^2 =0.269,1.339,P＆gt;0.05）.The expression rate of PAI-1 protein in endometriotic stro-mal cells in stage ⅢandⅣwas significantly higher than that in stage ⅠandⅡ（χ2 =5.432,P=0.020）.A positive as-sociation was observed between uPA and PAI-1 protein in endometriotic stromal cells （rp =0.311, P=0.039）.Conclu-sion：The overexpression of uPA and PAI-1 in endometriotic stromal cells might be of importance for the adherence ,inva-sion and growth ,which may result in endometriosis in the end .
