前列地尔联合雷公藤多甙片治疗糖尿病肾病蛋白尿的临床研究

摘　　要：目的观察前列地尔联合雷公藤多苷片治疗糖尿病肾病的疗效。方法将75例糖尿病肾病患者随机分为对照组、前列地尔组和联合用药组（对照组基础上加用前列地尔及雷公藤多苷片）。观察治疗前和治疗6、12周后24h尿蛋白定量、血白蛋白、肾功能、肝功能、血常规等变化，同时观察药物的不良反应。结果三组治疗前24h尿蛋白定量比较差异无统计学意义（P〉0．05）；治疗6、12周后各组尿蛋白均较治疗前降低，两治疗组组间比较差异均有统计学意义（P〈0．01），对照组组间比较差异无统计学意义（P〉0．05）；且联合治疗组治疗6周后与对照组治疗6周后相比，差异有统计学意义（P〈0．05）；联合治疗组治疗12周后与对照组治疗12周后相比，差异有统计学意义（P〈0．01）。其余指标如血白蛋白、血肌酐等变化差异无统计学意义。结论前列地尔联合雷公藤多苷片治疗蛋白尿效果明显，能明显降低糖尿病肾病患者蛋白尿，从而延缓’肾功能进展。Objective To observe the effect of alprostadil combined with tripterygium glycosides tablet on diabetic nephropathy. Methods Seventy-five patients with diabetic nephropathy were randomly divided into control group, alprostadil group and combination group （with alprostadil and tripterygium glycosides tablet added on the basis of the control group）. The changes of the 24 h urinary protein, serum albumin, renal function, liver function, blood routine before treatment and 6, 12 weeks after the treatment we~ observed, and the side effects of drugs were observed. Results Twenty-four hours urinary protein quantitative had no significant difference among the three groups betore treatment （P 〉 0. 05） ; After 6, 12 weeks of treatment, the urine protein were lower than those betore treatment, and the two treatment groups had significant differences （P 〈 0. 01 ）, but the control group had no significant difference （ P 〉 0. 05 ）. Compared with the control group, there was significant difference in treatment group after 6 weeks treatment （ P 〈 0. 05 ）, and there were obviously differences in treatment group after 12 weeks treatment （ P 〈 0. 01 ）. The changes of other indicators of serum ＂albumin, serum creatinine had no significant differences. Conclu sions Alprostadil combined with tripterygium glycosides tablet is very effective on DN proteinuria, which can significantly reduce proteinuria in patients with DN, thus delay the progression of renal function.
