摘  要:目的:观察评价离体条件下不同处理方式对牙周病患牙根面粗糙度及牙周膜细胞(PDLCs)在其表面粘附、增殖的影响。方法:将因重度慢性牙周炎而拔除的前磨牙和磨牙57个随机分为3组(n=19),分别用Gracey刮治器单纯手工刮治、手工刮治结合激光照射、手工刮治结合龈下喷砂的方法处理根面,用扫描电镜(SEM)观察表面形态、原子力显微镜(AFM)观察表面粗糙度后,将hPDLC接种于各组牙根片表面进行培养;分别于接种后1、6、12 h用SEM观察细胞粘附情况;培养后1、3、5 d MTT法检测细胞增殖情况。结果:SEM观察显示:Gracey组表面玷污层较多,存在器械划痕,激光组居中喷砂组最少;AFM观察显示:表面粗糙度以喷砂组最低,激光组最大(P<0.05);牙周膜细胞接种于根面6 h后激光组与喷砂组细胞伸展充分,Gracey组细胞仍呈皱缩状,12 h后3组细胞均充分伸展;细胞接种于根面1、3、5 d后,各组的细胞数量均随培养时间而增加,激光组增殖最明显,喷砂组次之,Gracey组最低,各时间点各组间两两相比P<0.05。结论:与单纯Gracey刮治相比,刮治结合激光照射、喷砂处理更有利于牙周膜细胞在根面的附着和增殖。AIM: To study proliferation of human periodental ligament cells (hPDLCs) on the root surface treated by different methods. METHODS:57 extracted teeth with advanced periodontal disease were divided into 3 groups( n = 19), the root surfaces were treated by Gracey scaling (group G), Gracey scaling plus YAG laser irradiation (group GYL) and Graeey scaling plus abrasive blasting (group GAB) respectively. The root surface morphology and roughness were observed by SEM and atomic force microscope (AFM). The attachment and proliferation of hPDLCs cultured on the root sufaces were observed by scanning electron microscope (SEM) and MTT assay respectively. RE- SULTS : The roughness of the root surface of group GAB was lower than that of group G and group GYL ( P 〈 0.05 ). The least smear layer was observed in group GAB and the most smear layer was observed in group G. 6 and 12 hours after culture the cell attachment in group GYL and group GAB was more extended than that in group G. 3 and 5 days after culture the proliferation of hPDLCs in group GYL was more obvious than that in group GAB and G ( between each 2 groups, P 〈 0.05 ). CONCLUSION : Abrasive blasting or YAG laser irradiation plus Gracey scaling is more effective than Gracey scaling in the treatment of root surface for the attachment and proliferation of hPDLCs.
