摘　　要：目的介绍临床药师对重度癌痛患者的药学监护实践。方法针对具体案例,参与个体化镇痛治疗方案的设计与调整,对药品不良反应及时监测并提出药学干预建议,保证临床合理用药。结果临床药师参与药物治疗,可发挥专业特长,协助医师调整用药方案,提高临床药物治疗水平。结论对重度癌痛患者的药学监护能提高患者用药安全性、合理性,提高治疗效果。Objective To describe the clinical pharmacists way in carrying out pharmaceutical care for cancer patients with severe pain. Methods Combining with the clinical experience, make the targeted treatment scheme with the physicians together and adjust the scheme according the condition change. Pay close attentions on the patients and make reasonable suggestions as to the adverse drug reactions to guarantee the rational use of drug. Results Clinical pharmacists can assist clinicians to adjust the therapeutic regimen from the perspective of pharmacists, and improve the level of clinical drug treatment. Conclusion Pharmaceutical care on cancer patients with severe pain is needed, as it can improve the safety and rationality of taking drugs so that improve the efficacy of the treatment scheme.
