急性缺血性中风中医证候特征与危险因素的相关性

摘　　要：目的探讨急性缺血性中风的相关危险因素与中医证候的关系。方法收集2005年3月至2006年10月共23家临床单位确诊为急性缺血性中风患者1841例,采用自拟调查表进行调查,包括患者的临床情况、患病情况、个人史、既往史等。将纳入的病例根据中风阳类证、阴类证辨证标准分为阳类证组757例、阴类证组1084例,记录两组患者危险因素,包括性别、吸烟史、饮酒史、性格易怒、嗜食肥甘、肥胖(BMI指数)、糖尿病史、高血压病史、既往中风病史、颈椎病史10项,并进行阴阳类证与危险因素相关性的Logistic回归分析。结果除外肥胖和颈椎病史,其他8个危险因素两组间差异具有统计学意义(P<0.05)。Logistic回归分析显示,性别、性格易怒、高血压病史这3个因素是影响阳类证辨证的相关因素(P<0.05),OR值均大于1,是易辨证为阳类证的相关因素。结论急性缺血性中风患者主要表现为阴类证,但阳类证组各危险因素影响较大,男性、性格易怒、高血压病史的患者易表现为阳类证。Objective To research the correlation between syndrome characteristics and risk factors of acute is chemic stroke. Methods Totally 1 841 cases of acute ischemic stroke were collected in 23 hospitals from March 2005 to October 2006. A Stroke Syndrome Questionnaire was used to investigate the clinical conditions, individual disease history and past medical history of patients with acute ischemic stroke. The patients were divided into the Yang-syndrome group of 757 cases and Yin-syndrome group of 1084 cases according to syndrome identification. The risk factors such as gender, smoking history, drinking history, irritability, fatty and sweet diet, obesity, history of di abetes, history of hypertension, history of stroke and history of cervical spondylosis were recorded. The correlation be- tween syndrome characteristics and risk factors was analyzed with Logistic regression. Results There were signifi cant differences between groups in 8 risk factors except obesity and history of cervical spondylosis （ P 〈 0.05 ）. The Logistic regression showed that 3 risk factors i. e. gender, irritability and history of hypertension were closely related to the identification of Yangsyndrome （P 〈 0.05） and the odd ratio （OR） values were greater than 1. Conclusion The common syndrome of acute ischemic stroke is Yang-syndrome and male, irritability and history of hypertension are closely related to the identification of Yang-syndrome.
