蛭龙活血通瘀胶囊结合中医辨证治疗缺血性中风急性期30例临床观察

摘　　要：目的观察蛭龙活血通瘀胶囊联合中医辨证治疗缺血性中风急性期患者的临床疗效。方法将45例缺血性中风急性期患者随机分为治疗组30例与对照组15例,两组患者均按照中医辨证给予中药免煎颗粒(风火上扰证给予天麻钩藤饮加减;风痰阻络证给予半夏白术天麻汤合桃红四物汤加减;痰热腑实证给予星蒌承气汤加减;阴虚风动证给予镇肝熄风汤加减;气虚血瘀证给予补阳还五汤加减),每日1剂。治疗组加服蛭龙活血通瘀胶囊每次3粒,每粒0.4g,每日3次。两组均治疗14天后观察临床疗效。治疗前后进行两组脑卒中患者临床神经功能缺损程度评分标准(CSS)评分、美国国立卫生研究院脑卒中量表(NIHSS)评分、日常生活活动能力量表Barthel指数(BI)评分、实际完成日常生活活动的改良Rankin量表(MRS)评分。结果治疗组临床疗效总有效率(93.33%)优于对照组(73.34%)(P<0.05)。治疗组治疗前后CSS评分、NIHSS评分、BI评分和MRS评分差异均有统计学意义(P<0.05或P<0.01),治疗组治疗后CSS、MRS评分较对照组明显降低,差异有统计学意义(P<0.01)。结论在辨证论治基础上配合使用蛭龙活血通瘀胶囊能明显提高缺血性中风急性期患者临床疗效,在改善神经功能缺损方面具有积极意义。Objective To observe the therapeutic effect of Zhilong Huoxue Tongyu Capsules [ capsules of Shuizhi （Hirudo） and Dilong （Lumbricus） to quicken the blood ] combined with TCM syndrome identification for acute ische mic stroke. Methods Totally 45 cases of acute ischemic stroke were randomized into the treatment group and con trol group, with 30 and 15 in each. Both groups were given free fried Chinese medicine granules according to syn drome identification one dose daily. The treatment group was added with Zhilong Huoxue Tongyu Capsules three times daily. The therapeutic effect was observed after 14-day treatment. The scores of Canadian Stroke Scale （ CSS）, Na tional Institutes of Health Stroke Scale （ NIHSS）, Barthel Index （BI） and Modified Rankin Scale （MRS） were evalu ated. Results The total effective rate of the treatment group and control group was 93.33% and 73.34% respec tively, with a significant difference between groups （P 〈 0. 05 ）. The scores of CSS, NIHSS, BI and MRS were signif icantly improved in the treatment group after treatment （P 〈 0. 01 ）. The scores of CSS and MRS were significantly improved in the control group after treatment （ P 〈 0.01 ）. Conclusion Zhilong Huoxue Tongyu Capsules com bined with TCM syndrome identification is effective for acute ischemic stroke and it can obviously improve the neuro logical deficit.
