中盖艾滋病项目以快速检测推动高危人群检测策略的效果评估

摘　　要：目的对中盖艾滋病项目延期设计的以快检推动高危人群检测策略进行评估。方法收集重庆、上海两个项目地区中盖艾滋病项目2011-2013年项目实施的核心指标和财务资料,从核心指标变化和经济学评价比较2013年快检项目实施与2011年和2012年的差异,从而评估延期项目策略的有效性。结果上海和重庆两个项目地区2013年检测人次分别为9 473人次和7 923人次,新发现阳性分别为616和668人;初筛阳性率分别为8.10%和8.67%,均低于2011年和2012年,差异有统计学意义;初筛阳性进行确证实验的比例分别为86.77%和97.52%,上海与前两年的比较差异无统计学意义,重庆较前两年有所提高,具有统计学意义;重庆2013年与2012年相比,初筛阳性进行免疫蛋白印迹法确诊实验的比例每增加1%,需要多投入26 634.65元;重庆和上海两地每发现1例阳性成本分别为2 102.40元和1 676.79元。结论延期项目以快检推动高危人群检测的策略现实可行,在今后的研究中应当拓展评估快检在改善随访和后期治疗的意义。Objective To evaluate application of HIV rapid testing among high risk group. Methods Core indicator and finance da- ta, which illustrated the program implemented in Shanghai and Chongqing from 2011 to 2013 was collected. Results A total of 9473 and 7923 tests were completed in Shanghai and Chongqing respectively in 2013; 616 and 668 new people living with HIV/AIDS （PLWHA） were detected respectively. After the primary screening, the percentages of HIV positive people were 8.10% and 8.67%, which were decreased when compared with 2011 and 2012 in statistics. Percentages of HIV positive people confirmed by Western Blot or alternative approaches were 86.77% and 97.52% respectively, but it was stable in Shanghai and increased in Chongqin. E- conomics evaluation showed that increasing 26 634.65 yuan input could increase 1 percentage of screening HIV positive results by Western Blot or alternative approaches in Chongqing. In 2013, the costs of identifying a new PLHWA in Shanghai and Chongqing were respectively 2 102.40 yuan and 1 676.79 yuan. Conclusions Rapid testing can promote HIV testing among high risk group, however, evaluation for HIV testing should extended to the follow up visit and therapy.
