华蟾素注射液联合化疗治疗胃癌疗效的Meta分析

摘　　要：目的系统评价华蟾素注射液联合化疗与单纯化疗治疗胃癌的有效性和安全性。方法采用Cochrane系统评价方法,计算机检索PubMed、中文科技期刊全文数据库、中国期刊全文数据库、万方数据库等,纳入含华蟾素注射液联合化疗治疗胃癌的随机对照研究(RCT)。由2名评价者独立评价并交叉核对纳入研究质量,对纳入的同质研究采用Rev Man5.0进行Meta分析。结果共13篇文献806例患者符合纳入标准。Meta分析结果表明,在提高近期有效率,改善生存质量,缓解化疗后Ⅱ度以上白细胞减少,减少病人胃肠道反应以及缓解病人疼痛方面,华蟾素注射液联合全身化疗优于单纯全身化疗,且差异有统计学意义(P<0.05)。但在提高生存率和中位生存期方面,华蟾素注射液联合全身化疗与单纯化疗比较差异无统计学意义(P>0.05)。结论华蟾素注射液对胃癌有一定的辅助治疗作用,主要体现在提高近期有效率和改善生存质量、减少不良反应等方面,但不排除与纳入文献质量较低、各种治疗方案纳入研究数量少和发表偏倚有关,期待更多设计合理、严格执行的大样本随机双盲对照试验提供高质量的证据。Objective To evaluate the effectiveness and safety of Cinobufacini injection combined with chemotherapy vs. chemo- therapy alone for gastric carcinoma（GC）. Methods We have searched the Cochrane Library,PubMed, CNKI, VIP and Wanfang Database. RCTs about Cinobufacini injection combined with chemotherapy for GC were included. 2 reviewers evaluated the quality of included trials independently. The Cochrane Collaboration software Rev Man 5.0 was used for meta - analyses. Results 13 RCTs were included in the meta - analysis. Results of meta - analysis showed that Cinobufaeini injection combined with systemat- ic chemotherapy was better than chemotherapy alone in the treatment of GC in respect of effective rate, the quality of life, grade Ⅱ lvukopenia, gastrointestinal reaction, relief of pain. There was statistical significance（ P 〈 0.05 ）. There was no statistical signifi- cance in improving survival rate and median survival time between Cinobufacini injection combined with systematic chemotherapyand chemotherapy alone（ P 〉 0.05 ）. Conclusion Cinobufacini injection is beneficial to the treatment of GC. However, consider- ing about low quality of included literatures and quantity limit and publish bias, more reasonable - designed and large - scale ran- domized double- blind controlled trials are needed.
