肠癌患者外周血T淋巴细胞亚群数量及临床意义

摘　　要：目的检测结直肠癌患者外周血淋巴细胞各亚群绝对值及比率以评估肠癌患者的免疫状态,并进一步分析其与肠癌患者临床特征的关系。方法收集167例肿瘤内科初次就诊的肠癌患者外周血,其中112例为根治术后患者,55例为晚期一线肠癌患者。同时收集166例体检中心健康人群的外周血,应用流式细胞术进行淋巴细胞亚群的检测。应用t检验及χ2检验进行统计分析。结果肠癌患者外周血CD4+T细胞绝对值较正常人显著下降;晚期肠癌患者CD4+T及CD8+T细胞绝对值较正常人显著下降,且相对根治术后患者,CD8+T细胞有下降趋势。此外,<59岁的晚期患者CD4+T细胞下降更明显。结论术后和晚期结直肠癌患者均存在免疫功能下降,为进一步了解肠癌患者的免疫状态和免疫治疗提供依据。Objective To evaluate the immune status of colorectal cancer patients through detecting the absolute value of lymphocyte subsets in the peripheral blood, and further analysis the relationship between the immune status variation and the patients＇ clinical features. Methods Peripheral blood of 167 patients that being treated in the First Affiliated Hospital of China Medical University were collected, including 112 cases with postoper- ative bowel cancer and 55 cases of first line advanced bowel cancer. A total of 167 healthy individuals admitted to medical centers were also recruited as control. Flow cytometry was used to detect the lymphocyte subgroups. Results CD4＋T in the peripheral blood of colorectal cancer patients was sig- nificantly decreased comparing with the normal controls. CD4＋T and CD8＋F in the peripheral blood of advanced colorectal cancer patients were signif- icantly decreased compared with the normal controls. There was no significantly difference of the absolute value of CD4＋T cells between advanced eolorectal cancer patients and postoperative cancer patients. CD8＋T cells have a downward trend. In addition, those advanced colorectal cancer pa- tients with age less than 59 patients showed more obvious decreased CD4＋T cells. Conclusion There are drawbacks of immune function in colorec- tal cancer patients, which can be applied as supporting data for auxiliary immunotherapy.
