摘 要:目的探讨二维斑点追踪技术(2D-STE)定量评价法舒地尔对糖尿病合并动脉硬化大鼠左心结构及功能的影响。方法 Wistar大鼠27只,随机分为对照组(NC组,7只)、造模组(PC组,10只)、法舒地尔组(DI组,10只)。NC组普通饲料喂养。PC组和DI组采用链脲佐菌素尾静脉注射加高脂饮食5周制备糖尿病合并动脉硬化模型,成模后分别给予生理盐水、法舒地尔4周。观测各组大鼠左室结构及心肌径向应变率(SrR)、圆周应变率(SrC)的收缩期应变率峰值(SrR-S和SrC-S)、舒张早期应变率峰值(SrR-E和SrC-E)及舒张晚期应变率峰值(SrR-A和SrC-A)。结果 PC组舒张末室间隔、左室后壁厚度及舒张/收缩末左室内径均较NC组及DI组增大(P<0.05),而NC组与DI组间差异无统计学意义(P>0.05)。各组间短轴缩短率的差异无统计学意义(P>0.05)。PC组心肌SrR、SrC与NC组、DI组相比明显降低(P<0.05);DI组SrR、SrC较NC组降低(P<0.05),心肌病变较PC组减轻。结论 2D-STE能够定量评价法舒地尔对糖尿病合并动脉硬化大鼠左室结构与功能的影响。Objective To establish diabetic and athemsclel^osis rat models, and study the effect of fasudil on left ventricular structure and function using two dimensional speckle tracking echocatdiography (2D-STE). Methods A total of 27 healthy Wistar rats, 6-8 weeks, were randomly divid- ed into nomlal eontrol group (NC group, n =7), positive control group (PC group, n =10), and drug intervention group (DI group, n =10). Tail vein injection of STZ was used to prepare diabetic rats models. High-fat diets were given for 5 weeks. For the drug intervention : dtaing the high fat feed, the DI group was administrated with intraperitoneal injection of fasudil with the dose of 10 mg- kg ~. d ~ for 4 weeks. The PC group was given in- traperitoneal injection of physiological saline with the same dose. After conventional echocardiography, the systolic radial strain rate (SrR-S) and cir- cunfferential strain rate ( SrC- S ), early diastolic radial strain rate (SrR - E ) and circumferential strain rate ( SrC- E ), late diastolic: radial strain rate (SrR-A) and circumferential strain rate (SrC-A) were measured using 2D-STE. Results Compared with NC and DI groups, PC group had in- creased end-diastolic interventricular septum thickness, left ventricular posterior wall thickness, and left ventricular end-diastolic/systolic dimension (all P 〈 0.05 ). No sigrfificant difference was observed between DI and NC groups (P 〉 0.05 ). There was no significant difference found among the 3 groups in left ventricular fraction shortening (P 〉 0.05 ). The SrR and SIC were significantly decreased in PC group compared to both NC group and D1 group (P 〈 0.05 ). The myocardial tissue was severe swollen, interstitial hyperemia and necrosis in PC group than DI group. However, SrR and SrC scores in DI group were still lower compare to NC group (P 〈 0.05 ).
