PPH在内痔术后少见并发症的诊治分析(附6例报告)

摘　　要：目的探讨吻合器痔上黏膜环切术(PPH)治疗脱垂痔少见并发症的发生原因及处理方法。方法回顾性分析6例PPH术后发生并发症患者的临床资料,总结手术并发症的发生原因及处理措施。结果 1例女性患者PPH术后吻合口局部形成直肠口袋征;1例女性患者吻合口形成巨大瘢痕状结节;2例女性患者吻合口上缘形成囊肿,其中1例囊液清亮,1例囊内物色白性状如牙膏;1例男性患者吻合口形成实性包块切开包膜后发现为粪团;1例女性患者术后出现排便困难,证实为吻合口黏膜套叠。均经再次手术治愈。结论本文6例并发症临床发生率低,荷包缝合不当是导致PPH术后并发症发生的主要因素,再次手术疗效确切。Objective To explore the etiopathogenisis and treatment of the rare complications after procedure for prolapsing hemorrhoids.Methods A retrospective analysis was made on 6 cases of the complications after PPH,reviewed the causes of surgical complications and treatment.Results Rectal pocket syndrome of local anastomotic stoma occurrenced in 1 cases of female patients with PPH ;1 cases of female patients with anastomotic stoma occurrenced a hugescar nodules; 2 cases of female patients with anastomotic stoma rim occurrenced a cyst,one cases of cystic fluid is clear,another case of sac for white characteristic such as toothpaste;1 male patients with PPH occurrenced a feces group; 1 female patients with postoperative defecation difficulties,confirmed anastomotic mucosa invagination.All were cured after second operation.Conclusion The occurrence rate of clinical complication with PPH is low,a failure of the purse-string suture is probably the main factor leading to complications after PPH,a good curative effect can be made by operation again.
