荆门城区母乳喂养儿体格发育纵向随访研究

摘　　要：目的：了解荆门中心城区母乳喂养婴幼儿体格发育特点，以提供城市母乳喂养儿童体格发育参考数据。方法借鉴WHO 2006年新标准制定方法，在荆门中心城区纵向随访200名0～36月龄儿童共24次，记录体重、身长/身高、头围3项指标，其中136名儿童完成24次随访，对这136名儿童的体格发育数据进行分析。结果婴幼期最大生长速率出现在生后3月内，体重增速2月龄＞1月龄＞3月龄，身长和头围增速1月龄＞2月龄＞3月龄；3月龄后，随月龄增长生长发育速度趋缓。与WHO 2006年标准比较，①体重：男童除出生至满月、女童除出生至2月龄外，其他年龄点均高于WHO标准；②身长：男童除出生至7天、满月、30～36月龄，女童除出生、21～36月龄外，其他年龄点均高于WHO标准；③头围：男童除7～14天、2月龄、36月龄，女童除出生至21天、24～36月龄外，其他年龄点均高于WHO标准。结论荆门中心城区母乳喂养婴幼儿生长发育轨迹与WHO 2006年标准一致，各年龄点体格发育指标达到或超过WHO 2006年颁布标准。Objective To understand the growth pattern of breastfeeding infants in the centre area of Jingmen and to provide references for physical development of breastfeeding infants in city .Methods According to Growth Monitoring Project issued by WHO in 2006, 136 breastfeeding infants were recruited by the project , and their weight , length, and head circumference were measured longitudinally from birth to 36 months of age in the centre of Jingmen .The monitoring frequency was 24 times in total.Results Breastfeeding infants showed their highest growth rate in the first three months since they were born .Monthly increment of weight gain reached peak at 2 months old, and then gradually declined .The increment of infant weight at one month old was greater than at 3 months old.The increment of the two remaining indicators-length and head circumference , continued to decline .Growth rate declined with age increasing after 3 months old . Compared to 2006 WHO Growth Standard , the weight of breastfeeding infants in Jingmen was higher at any age point except for the period from birth to one month in boys and from birth to two months in girls .The length of breastfeeding infants in Jingmen was higher except for the period from birth to seven days , one month old and from 30 months old to 36 months old in boys and at birth and the period from 21 months old to 36 months old in girls .Apart from second week after birth , two months old and 36 months old in boys and the period from birth to 21 days and the period from 24 months old to 36 months old in girls , head circumference of breastfeeding infants in Jingmen were higher than WHO Growth Standard .Conclusion The growth trajectory and development status of breastfeeding infants in the centre of Jingmen are in accordance with the Standard issued in 2006 by WHO, and infants’ physical increment value has almost exceeded it .
