子宫内膜厚度与体外授精-胚胎移植妊娠结局间的相关性研究

摘　　要：目的评估绒毛膜促性腺激素注射日子宫内膜厚度与体外授精-胚胎移植妊娠结局之间的相关性。方法回顾性分析1737个IVF-ET周期,按照HCG日子宫内膜厚度分为3组(A组:<8mm;B组:8-14mm;C组:>14mm),比较3组胚胎种植率、临床妊娠率及流产率等妊娠结局的差异。结果 3组的胚胎种植率分别为:16.0%、34.1%、18.6%,B组胚胎种植率虽高于A组和C组,但3组间无统计学差异(P>0.05);3组的临床妊娠率分别为:28.6%、46.7%、31.4%,B组临床妊娠率明显高于其他两组,有统计学差异(P<0.05),A组和C组临床妊娠率无统计学差异(P>0.05);3组的流产率分别为:15.9%、14.5%、14.7%,无统计学差异(P>0.05);结论注射HCG日子宫内膜的厚度影响了IVF-ET的治疗结局,厚度处于8-14 mm时将预示更好的临床结局。Objective： To assess the correlation between the endometrial thickness on the day of human chorionic gonadotrophin（ HCG） and the outcomes of in vitro fertilization and embryo transfer（ IVF- ET）. Methods： 1737 cycles of IVF- ET,which were divided into three groups according to the endometrial thickness on the day of HCG,were analyzed retrospectively.（ group A： 8mm; group B： 8- 14mm; group C： 14mm）. The difference of the outcome of embryo implantation rate,clinical pregnancy rate and abortion rate among the three groups were compared. Results： Embryo implantation rate of three groups were 16. 0%,34. 1%, 18. 6% respectively. Although embryo implantation rate of group B was higher than group A and group C,there was no statistical difference among the three groups.（ P 0. 05）. Clinical pregnancy rates of three groups were 28. 6%,46. 7%,31. 4% respectively. Clinical pregnancy rate of group B was obviously higher than the other two groups with significant statistical difference（ P 0. 05）. There was no statistical difference between group A and group C（ P 0. 05）. Abortion rates of three groups were 15. 9%,14. 5%, 14. 7% respectively,there were no statistical difference among them（ P 0. 05）. Conclusion： The endometrial thickness on the day of HCG has an effect on therapeutic outcome of IVF- ET,and its thickness between 8- 14 mm can predict a better clinical outcome.
