雌二醇的监测对预测早期先兆流产妊娠结局的重要性探讨

摘　　要：目的：探讨雌二醇（E2）的动态监测对预测早期先兆流产妊娠结局的价值。方法：回顾性分析108例6～9周单胎妊娠孕妇的临床资料，根据妊娠结局分为3组：正常妊娠48例为A组，先兆流产继续妊娠30例为B组，先兆流产妊娠失败30例为c组，动态监测血p—HCG、孕酮、雌二醇水平。结果：相同孕周A组和B组的β—HCG、孕酮、雌二醇明显高于c组，差异有统计学意义（P〈0．05）；A组与B组孕酮差异无统计学意义（P〉0．05）。A组与B组的β-HCG、雌二醇与孕周呈正相关性，孕酮水平与孕周的增长无相关性。监测过程中雌二醇水平呈平台（±10％）或下降状态提示预后不良。结论：早期先兆流产患者动态监测雌二醇对预测妊娠结局有重要意义。Objective： To explore the value of dynamic monitoring of estradiol value in predicting pregnancy outcome of threatened a- bortion. Methods： The clinical data of 108 single pregnant women of 6 -9 gestational weeks were analyzed retrospectively, then they were divided into three groups according to pregnancy outcome： group A （48 normal pregnant women）, group B （30 women of threatened abortion con- tinuing gestation） and group C （30 women of threatened abortion failing in pregnancy） ; the levels of blood β - human chorionic gonadotropin （β -hCG）, progesterone and estradiol were monitored dynamically. Results： During the same gestational weeks, the levels of β -hCG, progesterone and estradiol in group A and group B were statistically significantly higher than those in group C （ P 〈 0. 05 ） ; there was no statistical- ly significant difference in progesterone level between group A and group B （ P 〉 0. 05 ） ; in group A and group B, there was a positive correla- tion between 13 - hCG, estradiol levels and gestational weeks, but there was no conelation between progesterone level and increase of gestational weeks. In the process of monitoring, platform （ ± 10% ） or decrease of estradiol level indicated poor prognosis. Conclusion： Dynamic monito- ring of estradiol among patients with early threatened abortion has important significance in predicting pregnancy outcome.
