Study on Resting-state Functional Magnetic Resonance Imaging and Diffusion Tensor Imaging in the Patients with Mild Traumatic Brain Injury

摘  要:目的:利用静息态功能磁共振成像(fMRI)低频振幅(ALFF)联合弥散张量成像(DTI)评价轻度脑外伤(MTBI)患者脑功能活动;探讨ALFF异常脑区DTI的变化。方法:17例MTBI患者(MTBI组)与17例健康者(对照组)行静息态脑fMRI及DTI扫描。用入院时格拉斯哥昏迷评分及伤后6个月的简易精神状态评价量表(MMSE)评分,评估MTBI神经认知功能的变化。结果:静息状态下,MTBI组双侧额叶、小脑后叶ALFF升高,右侧丘脑、海马、脑干、双侧枕叶、左侧中央后回和右侧放射冠ALFF降低(P<0.005),双侧海马、左侧丘脑、右侧小脑的表观弥散系数降低,双侧额叶、脑干及左枕叶的各向异性分数降低(P<0.05)。两组间MMSE评分有统计学意义(P<0.01)。结论:静息状态下,MTBI患者脑功能与微观结构存在异常。Objective:To evaluate the brain activity of mild traumatic brain injury(MTBI)patients with amplitude of low-frequency fluctuation(ALFF)in resting-state functional magnetic resonance imaging(fMRI)combined with diffusion tensor imaging(DTI),and to study the change of DTI in abnormal ALFF regions. Methods:The resting-state fMRI and DTI were carried in 17 patients with MTBI(MTBI group)and 17 healthy persons(control group). The change of neurocognitive function with MTBI was evaluated by Glasgow Coma Scale and Mini-mental State Examination(MMSE)score after 6 months injury. Results:MTBI patients showed increased ALFF values in the bilateral frontal lobe and cerebellar posterior lobe activation,but decreased ALFF values in the right thalamus,right hippocampus,brainstem,bilateral occipital lobe, left post-central gyrus,and right corona radiate(P〈0.05). In the regions of interest,MTBI showed decreased FA values in the bilateral frontal lobe,brainstem,and left occipital gyrus,and decreased ADC values in the left thalamus,bilateral hippocampus and right cerebellar posterior lobe(P〈0.05). There was significant difference in MMSE scores between two groups(P〈0.01). Conclusion:Both brain functional and microstructural abnormalities exist in MTBI patients in resting state.
