血栓弹力图检测对指导临床合理输注血液成分作用的探讨

摘　　要：目的:探讨血栓弹力图(TEG)检测对临床患者合理使用血液成分的指导作用。方法:选择临床医生申请进行TEG检测的患者295例,其中有189例患者分别进行TEG检测和传统凝血项检测,选取TEG检测参数结果和凝血项指标进行相关分析,并对TEG检测结果所指导的临床用血成分与临床申请输注血液成分进行比较。结果:APTT、PT与R、K为正相关,r分别为0.379、0.326和0.255、0.239,P<0.01;Fg与K、Angle、MA呈显著相关,r分别为-0.289、0.391和0.584,P<0.01;PLT与K、Angle角、MA呈显著相关,r为-0.372、0.461和0.645,P<0.01。结论:TEG检测对临床患者合理使用血液成分具有指导意义。Objective：To discuss the guidance of thromboelastography （TEG） in rational use of blood components to clinical patients. Method： Analyze the TEG detection parameters and blood coagulation indexes of 189 patients out of the 295 patients, who was selected by clinician for TEG detection, and make comparison between the clinical use of blood components under the guidance of the TEG test results and the blood component of clinical application for transfusion. Then the following results were obtained. Result： ①APTT, PT and R, K had a signifi- cantly positive correlation. The coefficients of correlation, r, were 0. 379, 0. 326 and 0. 255, 0. 239, P〈0.01. ②Fg and K, Angle, MA had a significant correlation. The coefficients of correlation, r, were -0. 289, 0. 391 and 0. 584, P〈0.01. ③ PLT and K, Angle, MA had a significant correlation. The coefficients of correlation, r, were -0. 372, 0. 461 and 0. 645, P〈0. 01. Conclnsion：TEG detection in rational use of blood components for clinical patients may have guiding significance.
