摘 要:目的:探讨WADiana全自动配血系统在患者不规则抗体筛选中的应用价值并分析影响因素。方法:采用WADiana全自动配血系统对11 435例住院患者进行不规则抗体筛选。并将不规则抗体筛查阳性标本送市中心血站进行不规则抗体鉴定。结果:11 435例样本中,仪器共判读出不规则抗体阳性64例,经中心血站鉴定阳性为52例,阳性率为0.45%,假阳性12例。检出的抗体中特异性抗体占76.92%;非特异性抗体阳性占23.08%。影响检测结果的主要因素为纤维蛋白析出(占66.67%)。结论:WADiana全自动配血系统适用于临床患者不规则抗体的批量筛查,使试验过程标准化,检测结果准确、安全。但纤维蛋白析出、乳糜血以及仪器加样系统的携带污染等会导致假阳性结果,在日常检测中应引起重视。Objective:To explore the value of WADiana automated blood analysis system in irregular antibody screening and analyze the influential factors of experiment. Method: Using WADiana automated blood analysis sys- tem to screen irregular antibody in 11 435 hospitalized patients during the period from September, 2012 to March, 2013. Positive samples were sent to Ningbo Blood Center for identification of antibody specificity. Result: There were 64 positive samples in 11 435 patients detected by instruments. 52 cases were identified positive by Blood Center and the positive rate was 0.45 ~/'/0. Among them, specific antibody and non-specific antibody accounted for 76.92% and 23.08%, respectively. The false positive cases were 12 detected by instruments. The main factor affecting the test results was fibrin deposition accounting for 66.67 %. Conclusion:WADiana automated blood anal- ysis system could standardize the test process and be suit for batch detection, with accurate and safe results. But fibrin deposition, chylaemia and carryover of instrument adding system could lead to false positive results, which should be paid attention to in the daily detection.
