摘  要:目的:探讨具有江西汉族遗传背景人群中人类白细胞抗原(HLA)-B*1502与卡马西平(CBZ)所致Stevens-Johnson综合征/中毒性表皮坏死症(SJS/TEN)相关性。方法:在具有江西汉族遗传背景人群中,收集CBZ所致SJS/TEN患者19例,以及90例服用CBZ无过敏患者和80例健康对照者,采用序列特异性引物聚合酶链反应法进行HLA—B*1502的基因型测定.观察CBZ所致的SJS/TEN与HLA—B*1502基因型是否存在关联。结果:CBZ所致SJSfrEN组HLA—B*1502基因型携带率为100%,显著高于CBZ耐受组(15.5%)及健康对照组(18.75%),差异均具有统计学意义(P〈0.001)。CBZ耐受组HLA—B*1502基因型携带率与健康对照组比较,差异无统计学意义(生0.684)。结论:具有江西汉族遗传背景人群中.HIJA—B*1502与CBZ所致SJSfrEN有极强的相关性。Objective: To determine whether HLA-B*1502 allele is associated with carbamazepine-induced Stevens-Johnson syndrome and toxic epidermal neerolysis in Hanzu people in Jiangxi province. Methods: HLA-B*1502 allele genotyping was performed by a polymerase chain reaction-sequence specific primers (PCR-SSP) method in 19 Han subjects who had carba- mazepine-induced Stevens-Johnson syndrome and toxic epidermal necrolysis, 90 carbamazepine tolerant controls,and 80 healthy individuals. Results: The frequency of HLA-B*1502 allele in SJS/TEN patients was significantly higher than that in carba- mazepine-tolerant controls(100% vs.15.5%, P〈0.001) and normal individuals (100% vs.18.75%, P〈0.001). But the frequency in carbamazepine-tolerant controls did not differ significantly from that in healthy controls (P=-0.684). Conclusions: There is a strong association between HLA-B*1502 allele and carbamazepine-induced Stevens-Johnson syndrome, and toxic epidermal neerolysis in Hanzu people in Jiangxi province.
