摘　　要：目的研究少见的类似肿瘤表现的前列腺炎的多模态MRI表现。方法搜集2011年1月至2013年9月期间因前列腺疾病行MRI检查的288例患者资料,对其中影像学表现类似肿瘤而经病理证实为前列腺炎的18例患者的MRI表现进行回顾性分析。结果 18例均行MRI平扫和DWI,11例行增强扫描。MRI平扫表现为T2WI上外周带双侧弥漫性低信号8例(44.4%),双侧片状非结节状低信号2例(11.1%),多双侧对称,无明显占位效应16例(88.9%),结节状低信号3例(16.7%),其中局部外凸2例;DWI上稍高信号14例(77.8%),其中结节状稍高信号3例,正常4例;多期增强动脉早期多无明显强化7例,早期轻度强化4例,动脉晚期均匀强化6例,不均匀强化5例。结论类似肿瘤表现的前列腺炎的多模态MRI表现具有一定特异性,即前列腺外围带T2WI稍低信号,呈弥漫性或非结节状,DWI上呈无或轻度受限,动态增强多呈平台型强化。Objective To investigate multiparametric MRI features of rare prostatitis that has similar imaging features of tumor lesion. Methods During the period from Jan. 2011 to Sep. 2013 at authors＇ hospital, MRI scanning was carried out in 288 patients with prostatic diseases. Of the 288 patients, the diagnosis of prostatitis was pathologically proved in 18 with imaging finding being similar to that of tumor lesion. The MRI findings of the 18 patients were retrospectively ana- lyzed. Results Both plain MRI and DWI scanning were performed in all 18 patients, and additional contrast-enhanced scanning was carried out in 11 patients. Plain MRI showed that bilateral diffuse low-signal was seen at the peripheral zone of the prostate on T2 WI in 8 cases （44.4%）, and bilateral patch and non-nodular low-signal was observed in 2 cases （ 11. 1% ）. No obvious space-occupying effect could be detected in 16 cases （88.9%）. Other findings included nodular low- signal （n = 3, 16.7% ） and slight high-signal on DWI （n = 14, 77.8% ）. Normal finding was seen in 4 cases. Multi- phase contrast-enhanced MRI scanning revealed that no obvious enhancement at early arterial phase was found in 7 cases except for slight enhancement in 4 cases, while at delayed arterial phase homogeneous enhancement could be seen in 6 ca- ses and heterogeneous enhancement in 5 cases. Conclusion The prostatitis that has similar imaging features of tumor le- sion carries some certain characteristics on muhiparametric MRI, such as diffuse or non-nodular low-signal at peripheral zone of the prostate on T2 WI, slight high-signal on DWI, platform pattern enhancement on dynamic contrast-enhanced scan- ning, etc.
