孙氏手法改善退行性下颈椎失稳症疗效观察

摘　　要：目的:观察孙氏手法改善退行性下颈椎失稳症的临床疗效。方法:97例退行性下颈椎失稳症患者随机分为手法组49例和对照组48例。手法组采用孙氏颈椎手法治疗;对照组采用颈椎坐位牵引治疗,两组治疗时间均为2周。记录两组患者治疗前后颈部功能障碍指数评分(Neck Disability Index for Chronic Pain,NDI),对伴眩晕症状患者进行颈性眩晕症状与功能评估评分(Evaluation Scale for Cervical Vertigo,ESCV);所有患者治疗前后均拍颈椎功能位平片,测量患者失稳节段的水平位移(Horizontal displacement,HD)和角度位移(Angular displacement,AD);观察在治疗前后及2月后随访时两组的临床疗效。结果:①治疗后两组NDI评分比较,手法组低于对照组(P<0.05);ESCV评分比较,两组比较差异无统计学意义(P>0.05)。②治疗后两组HD、AD值比较,手法组均低于对照组,差异有统计学意义(P<0.01)。③在治疗后、2月随访时手法组临床总有效率分别为89.80%、87.75%;对照组为81.25%、77.08%,组间比较差异有统计学意义(P<0.01)。结论:孙氏手法可显著改善退行性下颈椎失稳症的临床症状以及失稳节段的椎体位移,其疗效优于牵引疗法。Objective：To observe the therapeutic effect of Sun＇s manipulation in the treatment of lower cervical vertebral de- generative instability. Methods： 97 patients with lower cervical vertebral degenerative instability were randomized divided in- to manipulation group （n= 49） and control group （n= 48）. Manipulation group were treated with Sun＇s manipulation thera- py. The control group were treated with cervical traction therapy. The Neck Disability Index for Chronic Pain （NDI） and the Evaluation Scale for Cervical Vertigo （ESCV） to patients with cervical vertigo symptoms were valued before and after two weeks treatment. The horizontal displacement （HD） and angular displacement （AD） were checked by X--ray determi- nation to instability cervical segment. The therapeutic effects were valued before and after treatment and two months follow -- up. Results：① After treatment, the NDI scores were significantly lower in the manipulation group than the control group （P〈0.05） ; ESCV scores between two groups were no significant difference （P〉0.05）. ② After treatment, the HD and AD of manipulation group were lower than control group＇s （P〉0.05） ③The total effective rate of manipulation group after treatment and during follow--up were 89.80% and 87.75%, while 81.25%, 77.08% of the control group ,with a signifi- cant difference between groups （P〈 0.01）. Conclusion.. Sun＇s manipulation therapy can significantly improve the clinical symptoms and cervical displacement caused by lower cervical vertebral degenerative instability, which more effectively than traction therapy and deserved clinical promotion.
