摘 要:目的:观察急性肺损伤大鼠的血浆和支气管肺泡灌洗液中脂连素(APN)水平变化,并探讨其与急性肺损伤(ALI)程度的相关性。方法:将45只SD大鼠,随机分为实验组(根据观察时间分为4h组、12h组、24组、48h组)与正常对照组,实验组小鼠给与气管内雾化吸入内毒素(LPS)0.5mL/kg(LPS 200μg/mL),对照组气管内吸入生理盐水0.5mL/kg,观察各组大鼠光镜下的病理改变并进行病理评分;采用酶联免疫吸附法(ELISA)分别测定各组大鼠血浆及BALF均化处理后APN和TXB2、6-keto-PGF1α的含量,并对BALF中炎症细胞分类计数及白蛋白测定。结果:实验组中4h、12h组血浆和BALF中APN、6-keto-PGF1α水平低于正常对照组,且血浆中APN差异有统计学意义(P<0.01),血浆中APN水平与ALI的病理学评分呈较高的负相关(r=0.936,r2=0.877,P<0.01);4h、12h组血浆和BALF中TXB2水平高于正常对照组(P<0.05);24h、48h组中APN、TXB2及6-keto-PGF1α水平与对照组无差异(P>0.05)。结论:APN参与了ALI的发生发展过程且血浆APN水平可反映ALI程度。Objective: To observe the variations of adiponectin in acute lung injury rats, and to investigate the relationship between the levels of APN and the degree of acute lung injury. Methods: 45 rat were randomly divided into 5 groups(each n= 9) :normal control group (0. 5 mL/kg NS instilled into the trachea via a needle) and experimental groups(0. 5 mL/kg LPS instilled into the trachea via a needle). Experimental groups were di- vided into 4h group, 12h group, 24h group, 48h group according to observation time. Rats were observed un- der light microscope for pathological changes and pathological scores. The concentrations of adiponectin and TXB2, 6-keto-PGFla in serum and bronchoalveolar lavage fluid (BALF) were determined by ELISA, and BALF was collected to assess the presence of poly morphonuclear neutrophils and albumin in BALF. Results: The concentrations of adiponectin and 6-keto-PGF1acute lung injury; bronchoalveolar lavage fluid; adiponectin; TXB2; 6 - keto - PGF1 α in serum and BALF in 4h group and 12h group were lower compared with those in normal control group, and the levels of APN in plasm have significant difference (P 〈 0.0l). What's more, the APN levels in plasm of the rat with acute lung injury were negatively correlated with the lung injury pathological score (r = 0. 942 ,r2= 0. 884 ,P 〈 0. 01). The levels of TXB2 in the plasm of 4h and 12h groups were higher than those of normal control group (P 〈 0. 05). However, the levels of adiponectin and TXB2, 6-keto-PGF1acute lung injury; bronchoalveolar lavage fluid; adiponectin; TXB2; 6 - keto - PGF1 α in 24h and 48h groups had no difference from those of normal control group (P 〈 0.05). Conculusion: Adiponectin palys a role in the development of acute lung injury, and adiponectin levels in plasm can reflect the degree of acute lung injury.
